Job Summary:

Play an important part in helping our patient to recover and feel better about themselves by providing therapy to maximize speech, language, cognitive and swallowing abilities. Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records. Document patient care in accordance with regulatory, licensing, payer and accrediting requirements.

Responsibilities and Duties:

· Play an important part in helping our patient to recover and feel better about themselves by providing therapy to maximize speech, language, cognitive and swallowing abilities.

· Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records.

· Instruct patient’s family or nursing staff in follow-through programs.

· Maintain equipment and work area in a safe and clean condition.

· Handle job responsibilities in accordance with the Company’s Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws.

· Evaluates patients within forty-eight (48) hours of physician referral.

· Develop effective treatment plans and obtain approval for services from referring physician.

· Treat patients according to the physician’s treatment plan and adhere to facility documentation guidelines.

· Communicate with supervisor and other health team members regarding patient progress, problems and treatment plans.

· Participate in Patient Care Conference, Utilization Review Meeting and Rehabilitation Conferences.

· Participate in in service training program for other staff in the facility.

· Record evaluations, daily treatment notes, weekly progress notes, HCFA therapy certs/POC forms per documentation guidelines.

· Record treatment charges per facility policy and procedure.

· Instruct patient’s families or nursing staff in maintenance program.

· Participate in discharge planning.

· Secure necessary durable medical equipment and adaptive equipment for patients to facilitate independence.

· Comply with the American Speech Language and Hearing Practice Act for the state, and the facility policy and procedures.

· Prepares, maintains, and disseminates patient records and reports as mandated by regulatory agencies, physicians, and at the written request of the patient while maintaining patient confidentiality when corresponding with occupational therapists and other healthcare professionals.

· Provides treatment in accordance with established standards of practice, department procedures, and productivity standards.

· Consistently demonstrates sound judgment in evaluation, planning implementation, and follow-up of resident therapy programs.

· Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.

· Maintains a current knowledge of law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are following applicable regulations.

· Attends and participates in scheduled training, education classes and meetings as required.

· Document patient care in accordance with Rehab Care, regulatory, licensing, payer and accrediting requirements.

Minimum Experience and Qualifications:

· Licensed Speech Therapist/Language Pathologist

· Graduate of master’s level program in speech language pathology/therapy

· Current CPR/First Aid License

· Long term care or 3 years of direct programming experience with older adults

· Valid state driver’s license and clean driving record
