We are a well known SUCCESSFUL Home Health /Home Care Agency located in the Chicago Land area We are in need of a Home Health RN (Registered Nurse) to conduct patient home visits in the SOUTH SUBURBS Competitive pay and flexible hours!

THE PAY IS $75.00 FOR EVERY START OF CARE VISIT and $65.00 FOR EVERY REVISIT
MUST HAVE A MINIMUM OF 6 MOS EXPERIENCE WORKING AS A HOME HEALTH RN

Home Health Nurse RN Responsibilities:
- Completing nursing process in delivery of patient care which includes: assessment, planning, implementation, and evaluation of patient outcomes.
- Total oversight of patient from admission to discharge.
- Developing plan of care based on findings.
- Serving as patient’s advocate and maintaining ongoing communication with physician.
- Timely completion of all skilled nursing documentation
Qualifications:
- Current Illinois State RN License
- Valid Drivers License and Reliable Vehicle
- MINIMUM of 6 months Experience as a HOME HEALTH VISITING RN
- Must know OASIS
- Must be RELIABLE and DEPENDABLE
- VALID CPR CERTIFICATION
Pay: $65.00 – $75.00 per hour
