Physical Therapist Assistant: On Call – Freedom Village Holland/Life Care Services – Holland, MI

GENERAL SUMMARY: Provides and directs Physical Therapy services to patients including screening, assessment, treatment, program planning and implementation. Functions under physicians’ orders and adheres to applicable principles and practices of physical therapy, community policies and procedures and state regulations.

PRINCIPLE DUTIES: : 1. Optimizes the physical abilities and skills of patients by performing or supervising therapy staff in the following activities: initiating referrals, evaluating and treating appropriate patients; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient’s family, the staff and other professionals as appropriate. 2. Establishes individual patient plan of care to optimally address patient-specific goals and toe comply with service delivery parameters in accordance with the resident assessment, care contract or other guidelines specific to long-term care. Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve results. 3. Develops treatment plan with patient, family and other team members. Implements and conducts treatment as outlined in the treatment plan. Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders. Adapts treatment to meet the individual needs of the patient. 4. Accesses the patient’s record and records results of screenings, physician orders, discharge summaries and other information as required in the Community’s medical record system. Documents results of patient’s assessment, treatment, follow-up and termination of services following standards established by the Center for Medicare and Medicaid Services and the Community. 5. Develops appropriate home program to maintain and enhance the performance of the patient in his/her own environment. 6. Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains patient, family and appropriate staff in the use or application of the device, orthotic or prosthetic. 7. Refers patients for treatment to Physical Therapy Assistants (PTA) by completing evaluation and initial plan of treatment and communicating necessary and sufficient information. Supervises and directs PTA in the implementation of the treatment plan. 8. Reviews the quality and appropriateness of the total services delivered and of individual physical therapy programs for effectiveness and efficiency, using predetermined criteria. Submits billing, payroll and expense documentation in an accurate and timely manner. Assists in the collection of statistics as directed. 9. Adheres to Community policies and procedures. Complies with all Infection Control, Universal Precautions, and OSHA standards for healthcare professionals. 10. Assumes responsibility for ongoing continuing education and professional development. Attends inservice training as well as provides education to members of the patient care team and members of the community.

QUALIFICATIONS: Bachelor of Science in Physical Therapy from an accredited program. Successful completion of the National Certification Examination for Registered Physical Therapist. Valid state license in practicing state. Previous physical therapy experience preferred.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Must be able to exert 20 to 50 pounds of force occasionally and/or 10 to 20 pounds of force frequently to move objects, equipment and/or patients. Regularly required to stand, walk, sit, stoop, kneel, bend or crouch; use hands to manipulate tools, equipment, or control; reach with hands and arms, balance, life and perform medium to maximum transfer assists with patients. The employee is required to have visual and hearing acuity sufficient enough to assess patient safety and ability. The employee works mostly indoors, and may be exposed to bloodborne pathogens.
