Licensed Social Worker – MCN – Houston, TX

MCN is looking for an experienced, knowledgeable LCSW/LMSW/LBSW

*******************This is a remote position for licensed Texas social workers only**********

The Community Health Case Manager will serve mostly pregnant women and children with special needs for birth to 20 years old receiving Medicaid benefits. The case manager will have knowledge of finding and providing resources to clients in the scope of practice. The case manager will also serve a liaison between the clinical director and all program coordinators. The position is only for an LBSW/LMSW or LCSW only!

All other professionals are not able to be considered for this specific position.

ESSENTIAL DUTIES, RESPONSIBILITIES

90% of all case management cases will be home visits or telehealth, the case manager must feel comfortable in providing services in clients’ homes and speaking on camera.

Prospective candidates must possess and understand the nine phases through which case managers provide care to their clients:

Screening, Assessing, Stratifying Risk, Planning, Implementing (Care Coordination), Following-Up, Transitioning (Transitional Care), Communicating Post Transition, and Evaluating

Provides direct services to individuals on caseload including completion of comprehensive assessments, draft service plans, provide accurate treatment plans, review case progress and determine case closure.

All services are provided in a safe, timely, effective, efficient, equitable, and client-centered approach.

Referral, Linkage, and Advocacy while helping clients achieve wellness and autonomy.

Ensure authorization for services.

Develop measurable objectives and goals agreed upon by the individual.

Document all attempts to involve individual and service providers.

Follow-up with assigned individuals who miss an appointment. Document attempts to contact the individual.

Document clinical services by close of the next business day.

EDUCATION, LICENSE, and EXPERIENCE

Must-Have a Valid Texas Social Work / RN License: LBSW, LMSW, LCSW, RN

At least two years of related experience and/or training in the field of mental health preferred.

At least two years in the acute care setting

Must have valid Texas Drivers License and good driving record.

Must be able to pass a criminal history check.

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge and experience in providing case management and skills training.

Ability to assess individual needs, coordinate services, and to communicate effectively.

Knowledge of the social services system and the ability to connect individuals with appropriate services.

Ability to write clearly and professionally.

Ability to think clearly and logically in order to deal with various types of situations.

Understands and implements trauma-informed care practices.

Ability to work independently and know when to reach out for additional support.

Ability to handle conflict in a professional and ethical manner.

Demonstrates effective problem-solving skills

Pay: $22.00 – $45.00 per hour
