Position Summary: *
The school health center nurse practitioner is responsible for the assessment and direct care of patients/students from preschool ages to 18 presenting to the clinic for appointments. The nurse practitioner under partial supervision of a collaborative physician to safely implement an individualized plan of care designed to meet the healthcare needs of each patient. The clinical staff will provide education to patients regarding their healthcare needs and preventive care information appropriate for age, sex, and health status.

Essential Functions (Responsibilities/Accountabilities): *

  • Performs examinations, orders and interprets diagnostic testing, and review results with patients
  • Speaks with patients and their families to better understand their concerns, needs, conditions, and medical histories
  • Establishes care or condition management plans, including prescribing medications or other treatments
  • Keeps records regarding patient history, condition, treatments, and progress
  • Collaborates with physician if needed based on patient condition
  • Practices in accordance of IDPH regulations and maintains HH employee requirements
  • Utilizes evidence-based guidelines to practice
  • Works to meet quality measures set forth by the facility
  • Ensures continuous flow of communication with leadership of Horizon Health and the school district to enhance quality of healthcare services to patients

Position Requirements: *

  • Master’s Degree or Doctoral Degree as a Family Nurse Practitioner or Pediatric Nurse Practitioner
  • Current Illinois License
  • Current BLS certification
  • Mental Health experience preferred
  • Ability to communicate verbally and in writing
  • Good nursing practice skills
  • Ability to perform complex and varied tasks in a fast-paced work environment subject to stress and interruption
