Responsibilities:
- Interview patients to identify their medical and social service needs.
- Educate patients about their condition, medication, and give specific instructions.
- Develop a care plan to address their personal health care needs.
- Respond to patient’s questions and expressions of concern.
- Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division’s philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations.
- Work as part of a team with other members of the interdisciplinary team.
- Locate and contact providers of medical care.
- Facilitate the coordination of social services for patients.
- Perform other duties as assigned.
Requirements:
- Medical terminology or background
- Experience in care coordination, case management, or clinical practices
- Strong analytical thinking and ability to handle multiple tasks concurrently
Preferred:
- Managed health care platforms and documentation experience
Job Type: Full-time
Pay: $17.00 – $21.00 per hour
