Lead Care Manager – Serene Health – Tulare, CA

Responsibilities:

  • Interview patients to identify their medical and social service needs.
  • Educate patients about their condition, medication, and give specific instructions.
  • Develop a care plan to address their personal health care needs.
  • Respond to patient’s questions and expressions of concern.
  • Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division’s philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations.
  • Work as part of a team with other members of the interdisciplinary team.
  • Locate and contact providers of medical care.
  • Facilitate the coordination of social services for patients.
  • Perform other duties as assigned.

Requirements:

  • Medical terminology or background
  • Experience in care coordination, case management, or clinical practices
  • Strong analytical thinking and ability to handle multiple tasks concurrently

Preferred:

  • Managed health care platforms and documentation experience

Job Type: Full-time

Pay: $17.00 – $21.00 per hour
