Job Summary

eCatalyst?s Remote Clinical Documentation Specialist (CDS) reports to our Director of Clinical Documentation Improvement and serves our valued clients nationwide. CDS?s are responsible for concurrently reviewing and analyzing inpatient or outpatient encounters to improve the overall quality, accuracy, and completeness of clinical documentation. CDS?s collaborate with providers and other healthcare team members to improve clinical documentation that results in accurately captured diagnoses, procedures, and treatments. This position works closely with Coding to support the appropriate patient severity of illness and risk of mortality. CDS?s work in collaboration with hospital departments to support efforts that justify patient admissions, medical necessity, continuity of patient care and other clinical documentation requirements.

Knowledge, Skills & Abilities:

  • Familiarity with MS-DRGs, APR-DRGs, the Inpatient Prospective Payment System (IPPS) and the Outpatient Prospective Payment System (OPPS)
  • Reviews medical records to identify the most appropriate principal diagnosis and assigns a working MS-DRG.
  • Performs initial reviews, concurrent reviews and retrospective reviews to ensure the MS-DRG accurately reflects the principal diagnosis and all comorbid conditions after study.
  • Completes DRG reconciliation of CDI assigned vs. Coding assigned MS-DRG
  • Requires exceptional critical thinking and communication skills.
  • Must possess advanced knowledge of a wide range of medical disciplines including anatomy and physiology, pharmacology, pathophysiology.
  • Knowledge and experience in writing effective queries leveraging the AHA Coding Clinic and AHIMA Query Practice Brief guidelines.
  • Must possess effective interpersonal skills to interact positively and collaboratively with Providers, coders, HIM Department, Quality Department, and other stakeholders.
  • Able to educate physicians, non-physician clinicians, nurses and coding staff on an ongoing basis regarding documentation opportunities, coding and reimbursement issues and relevant quality and performance improvement opportunities
  • Requires advanced analytical skills to interpret clinical documentation and write effective Provider queries.
  • Works collaboratively with Physician Advisors to ensure positive program outcomes
  • Requires solid computer skills (searching, navigating, composing queries, sending emails, etc.) to successfully navigate electronic medical records using a computer.
  • Requires the ability to read, write and speak effectively in English.
