POSITION: Charge Nurse

DEPARTMENT: Health Care Center

POSITION SUMMARY:

The Charge Nurse functions as a leader of the nursing team on an assigned floor and for an assigned shift, coordinates the total care of the residents, and directly supervises the CENAs.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

A. Administrative Responsibilities

1. Assist in planning and supervising resident care in accordance with the instructions of physicians, established Health Services policies and procedures and the employee manual.

2. Supervise nursing personnel in assigned area to ensure that Health Services policies and procedures and employee manual are followed, and that all nursing personnel adhere to their job descriptions.

3. Keep physician informed regarding the general condition of each resident as appropriate and inform him/her of a resident’s death.

4. Inform Unit Manager/Supervisor when significant changes occur in a resident’s condition.

5. Assist in monitoring seriously ill residents. Notify Unit Manager/Supervisor and/or Health

Service Director/Assistant Health Services Director of a change of condition.

6. Consult with the physician regarding resident medical needs as appropriate, and keep Health

Service Director/Assistant Health Services Director and/or Unit Manager/Supervisor informed.

7. Coordinate daily work assignments of nursing personnel and supervise staff to ensure that they fulfill their responsibilities and are accountable in performing assigned tasks.

8. Encourage and assist nursing staff to coordinate nursing services with other resident services to ensure continuity of care.

9. Work at creating an environment that will foster a spirit of cooperation and efficiency on the unit.

10. Review residents’ complaints and/or grievances and inform Unit Manager/Supervisor.

11. Ensure that all nursing personnel are informed of residents’ rights and responsibilities, and all residents are treated with fairness, dignity and respect.

12. Assume administrative responsibility for the unit and for its general appearance.

13. Attend meetings of HCC as required and/or requested.

14. Ensure that all nursing personnel operate equipment safely. Monitor personnel to ensure that supplies are used properly and efficiently.

15. Attend report at the beginning and end of shift.

16. Meet with shift personnel on a regularly scheduled basis to assist in identifying and correcting problem areas and/or improving services.

17. Participate in the planning and determining of staffing needs. Keep Unit

Manager/Supervisor informed about the unit and its needs.

B. Resident Care Responsibilities

1. Make daily rounds to residents in order to observe and evaluate their physical and emotional condition.

2. Encourage residents, as far as is practical, to participate in the development and review of their own plan of care.

3. Maintain confidentiality regarding all resident care information.
4. Assume responsibility for the preparation and distribution of medication and monitor all narcotics in the emergency boxes.

5. Assist staff with resident care as needed and appropriate, this includes hands-on care.

6. Possess knowledge and skills for I.V. Therapy, EKG, foley care, Hoyer lift, and other specialized care.

7. Follow the established Health Services policies for body mechanics and lifting.
C. Observation, Reporting and Documenting

1. Evaluate the work performance of assigned personnel and monitor the progress of new nursing personnel, especially during their in-training period. Discuss recommendations with

Unit Manager.

2. Ensure that Health Services dress code is observed by all nursing personnel.
3. Complete accident/incident reports as needed and inform Unit Manager/Supervisor.
4. Ensure that all record keeping, written reports, charting, written records, and other clerical duties are up-to-date and completed professionally.

5. Prepare written updates of residents’ conditions on the Progress Notes and other official records. Updates include the following: admissions/discharges, abnormal tests, change of condition, and/or other pertinent information.

6. Utilize computer programs for documentation, as indicated.
7. Ensure that any disciplinary action is administered fairly, carefully documented and communicated to Unit Manager/Supervisor.

D. Supplies, Equipment, Maintenance

1. Determine the need for special equipment and consult with and make recommendations to the Unit Manager regarding such equipment.

2. Routinely check oxygen and other supplies to make sure they are adequate for an emergency.
3. Order floor supplies as necessary for personal, nursing, and/or medical care from Central

Supply.

4. Ensure that all maintenance problems are reported to Maintenance Supervisor.
E. Get along well and work effectively with co-workers and others in performing job tasks.

F. Perform other duties of this position or related positions as may become necessary or as directed.

ACCOUNTABLE TO: Unit Manager

QUALIFICATIONS:

1. A nursing degree from an accredited college, university or school of nursing, or a graduate of an approved RN/LPN/LVN program.

2. Experience as a charge nurse in a hospital, long-term facility, or other related health care facility preferred.

3. Training in rehabilitative and restorative nursing practices in gerontology preferred.
4. Current, unencumbered Michigan license to practice as an RN or an LPN.
5. Knowledge of current nursing and medical practices and procedures.
6. Communication skills to present information and ideas essential to supervisory duties and to maintain good relationships with staff and medical personnel.

7. Ability to plan, organize, develop and implement programs, goals, objectives, policies and procedures necessary for quality care.

8. Patience, tact, enthusiasm, cheerfulness, and a willingness to work with difficult residents.
9. Willingness to seek out new methods and principles and incorporate them into existing nursing practices.

10. Flexibility to accommodate variable eight-hour responsibility.
11. Verbal ability as required to understand instructions, medical terminology and concepts, and to communicate with residents and HCC staff. Writing ability to keep accurate records.

12. Ability to cope with the mental and emotional stress of the position.
13. Good general health and emotional stability.
14. Ability to relate to and work with the ill, disabled, elderly, emotionally upset and, at times, hostile residents within the facility.

15. Ability to lift fifty (50) pounds or more.
16. Ability to lift, push, pull, bend, stoop and move equipment, supplies, etc. throughout the day as necessary.

17. Ability to speak, read and understand the English language.
18. Possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so the requirements of this position can be fully met.

19. Ability to maintain good relationships with others, regardless of personal preferences.
