Physical Therapist Home Health Job description:

Rehab Accomplished is a therapist-owned staffing agency serving Dallas and surrounding areas. We provide physical therapy, occupational therapy, and speech therapy services. Our mission is to provide quality and evidence-based intervention and make difference for others.

We will love to hire an experienced PT and new Grads as well. Training, and strong support will be provided as needed. At some point, all of us are new grads and I believe only a therapist can understand and cater to another therapist’s work needs so contact us and speak directly with an experienced therapist for further questions.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are normal for this position. This list is not to be construed as exclusive or all-inclusive.

  • Under the direction of the licensed physical therapist, optimize the functional abilities and skills of patients.
  • Work directly with patients to achieve maximum physical recovery
  • Carry out skilled therapy interventions with the individuals being served, consults with physical therapist regarding the individuals treatment and progress.
  • Record and document patient care services
  • Report on patient progress through appropriate written documentation. Follow all established policies and procedures for documentation regarding timelines and form completion.
  • Analyze tasks and adapt the environment, tools, materials, treatments to the needs of patients and their socio-cultural context.
  • Determine appropriateness of patient’s response to treatment and communicate findings to the Physical Therapy Supervisor concerning modifications to plan of care.
  • Interact with all patients, families, customers, visitors and fellow employees in a mature, responsible, courteous manner to ensure a positive and professional environment.
  • Attend care conference meetings and interdisciplinary rounds at the request of Physical Therapy Supervisor to discuss patient progress and future needs with other professionals involved in care of the patient.
  • Coordinate and prioritize own schedule in relation to demands of patient treatment, patient treatments with other professionals and other departmental responsibilities.

Qualifications/Skills:

  • License to practice as a Certified Physical Therapy Assistant in the State of Texas required
  • Current American Heart Association Healthcare Provider CPR / BLS Certification required
