We are looking for hard-working, caring, flexible and technologically proficient clinicians to join our growing team! Our Home Care Visit Nurse is responsible for traveling around Minneapolis and the surrounding areas to provide exceptional care to patients in their homes under the direct orders of a licensed physician. The Visit Nurse is responsible for assessing, documenting and assisting with the coordination of treatment for each patient’s individual needs.
Nurses will spend between 30-45 minutes with each patient seeing anywhere from 5-6 patients a day (when working full time) performing catheter care, IV Infusion, wound care, ostomy, medication management, etc. and then reporting any updates in patient status to the primary physician.
We are currently looking for visit nurses to see patients in or around the following areas in a full or part-time position:
- Brooklyn Park
- Brooklyn Center
- Maple Grove
- Anoka
- Plymouth
- Minneapolis
- Shakopee
- Burnsville
- Minnetonka
Specific Job Duties include but are not limited to:
- Operate efficiently in DeVero (electronic charting) and Microsoft Outlook; must be technologically proficient
- Educate patients and caregivers regarding process, nutrition, medication, self-care and other areas of home care
- Provides accurate and timely documentation at point of service
- Communicate information regarding patient’s condition or needs to the physician or other persons as necessary or appropriate to implement and facilitate patient’s plan of care
- Evaluate outcomes of the implemented plan of care
- Develop individualized patient care plans to address specific care or services to be provided to the patient per physician’s orders
- Assists in on-call coverage or scheduled visits to meet patient needs during non-office hours
- Act as a role model of AMHC’s values and mission
- Other duties as assigned.
Experience:
- Current MN LPN License in good standing
- 1-3 years of Skilled Home Care, TCU or Hospital experience preferred but not required
- Phlebotomy experience a plus
- Ability to multi-task and handle multiple caseloads simultaneously
- Must be able to work independently and as part of a team
