Lead Registered Nurse – WECARE TLC, LLC – Chambersburg, PA

The primary functions of the Health Center Lead RN (LRN) are to manage the day-to-day operations of their assigned clinic, assure that patient care coordination occurs in a timely and cost-effective manner, assist providers and provide nursing coverage as needed and engage We Champion for those members identified with higher health risk factors. The LRN, is empowered to create and maintain an atmosphere of a patient-centered medical home model. As the leader within their clinic, the LRN may have frequent interaction with the client contact. LRN will be responsible for meeting expectations of monthly clinic audit and other performance outcomes.

Must have a Certificate from an accredited Nursing School or Associate’s degree in Nursing (ASN) required. Bachelor’s degree in Nursing (BSN) preferred. Currently a state licensed Registered Nurse. Experience as a Registered Nurse in a physician practice, occupational health clinic, hospital department, or other ambulatory care clinic highly preferred. Past experience supervising clinical staff. Experience and knowledge of chronic diseases and treatment. Knowledge of Internet, Spreadsheet, and Word Processing software required. Experience on EMR systems. DOT certified.
