As an MDS Nurse for a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to:

Essential Job Functions:
Review residents admitted and compare physician orders with each resident’s plan of care to assure appropriate
implementation of the physician’s plan of care.
Schedule routine MDS assessments (initial, quarterly, annual) to maintain compliance with regulatory time frames.
Schedule ARD for all routine MDS assessments, following the most current RAI User Manual instructions, with input
from the Interdisciplinary Team.
Schedule change in status assessments as instructed.
Complete Re-entry and Discharge tracking MDS within 7 days of the event.
Communicate weekly team meeting date, time, and the following to team members:
Resident name
MDS assessment type (Initial Quarterly, Annual, Change in status, PPS)
ARD
RAPs due (as applies)
Completion due date
Communicate schedule to resident and appropriate family members/legal guardian
Assist with and/or complete data entry onto the computerized MDS record as directed by the RN Coordinator
Print appropriate MDS/RAP notes prior to the care plan meeting
Obtain signatures on completed MDS
Notify RN if difficulty obtaining signatures for appropriate follow up
File completed MDS in resident’s clinical record
Electronically submit completed MDS’ to the state on a weekly basis
Retrieve and maintain validation reports with each submission, discuss errors with the RN
Maintain current copies of informational updates on the state’s web site
Maintain current copies of the analytic and QI/QM reports

Distribute copies of informational updates, current QI/QM reports with administrator, DON, RN Coordinator, etc as instructed
Review reports of timely completion of resident MDS assessments and care plans with recommendations of follow up action to the MDS Coordinator.
Participate in patient care plan conferences.
Complete reports and assignments accurately and adhere to established time schedules.
Establish open lines of communication between all nursing personnel and consistently take follow-up action as necessary.
Obtain the resident assessments from pertinent departments.
Put information from all departments’ assessments into the MDS computer system and obtain MDS and RAPS as needed.
Identify special nursing problems and emergency situations and initiate an immediate plan of action according to the nursing home policies and procedures.
Assist in the admission/transfer and discharge documentation of residents according to federal and state guidelines.
Follow all federal and state regulations regarding the currently approved MDS and keep updated as changes occur through continuing education.
Act as a Charge Nurse and cover the floor/various shifts as needed for the best interest of the facility.
Find and attend continuing education programs appropriate to this position.
Maintain records of appropriate documentation according to federal, state and local regulations.
