Long-term contract position working in the community, homes and at the agency office with individuals and families to address mental and behavioral health illness in an effort to enhance their quality of life. Services provided are to be individualized and based upon detailed needs assessment. Provide services to assist clients with learning and developing independent living skills, social skills, illness management and coping strategies. Support is also to be provided to clients with accessing and coordinating benefits, additional services, and engaging community-based resources. Telehealth services are utilized but some cases may require in-person engagement.
