The Position:

This position is onsite and is responsible for performing assigned nursing care according to an established Plan of Care and monitors the performance of home health aide staff under the direction of the RN Case Manager. Typically visiting 5-6 clients per day.

Responsibilities include:

  • Promotes and maintains patient health through teaching appropriate rehabilitative measures
  • Assists patients in learning appropriate self-care techniques
  • Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals
  • Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks
  • Documents appropriately and timely in Home Health software system

Schedule: Weekdays – Day time hours – NO WEEKENDS!

Requirements:

  • LPN license required
  • Home Health experience is a plus
  • Possess a valid driver’s license with a good driving record and have reliable transportation
  • The ability to pass a background check
  • Strong verbal and written communication skills
  • Basic computer skills
