Swedish Medical Center is looking for an Opioid Care Navigator (1.0 FTE, Variable Shift) to work for Outpatient Psych at Swedish Edmonds in Edmonds, WA.

SCHEDULE: Monday – Friday; 8-4:30pm PST. NO WEEKENDS or ON-CALL scheduling.
Care Navigator provides clinical case management functions via a collaborative process that transitions patients to optimal clinical outcomes while assuring safe, cost-effective care.
In this position you will:
- Maintain a collaborative relationship with the healthcare team and community Opioid Treatment Network (OTN) to ensure the best possible coordination of care for individuals throughout their continuum of care.
- Conduct behavioral health and/or substance use disorder screenings and assessments, including, but not limited to the Columbia Suicide Scale.
- Coordinate data entry as required by OTN grant contract.
- Assist individuals with resources available throughout the OTN to assist with program outcomes.
- Communicate with OTN agencies, providers and other community agencies in support of linking individuals to MAT services.
Qualifications:
Required qualifications for this position include:
- Master’s degree in Social Work or Counseling
- Licensed Social Work Associate – Independent Clinical (LSWAIC) or Licensed Mental Health Counselor;
- Licensure as an Independent Clinical Social Worker (LICSW) with the Washington State Department of Health under Chapter 18.225 RCW is preferred and may be substituted
Preferred qualifications for this position include:
- ACM/CCM certification
- One (1) year directly related work experience social services in a heath care setting
- Experience in crisis intervention
