Under the direction of a licensed social worker, provide care coordination and comprehensive case management to health home clients with a range of chronic medical and mental health conditions and/or substance abuse. Duties include care coordination, community outreach, referrals to community-based services, client engagement, advocacy, health promotion and field work. Assist clients with meeting their health care goals by gathering data and completing and updating basic assessments and plans of care per DOH regulations. Duties will also include processing forms and data entry in state required computer systems.
Must be organized and flexible to meet deadlines and prioritize competing priorities. Excellent supervision and educational opportunities available.
Minimum Bachelors Degree
Bilingual Spanish preferred.
