Nurse Practitioner – New Hampshire Detox and Rehab – Dublin, NH

tle: Family Nurse Practitioner

Reports To: The position reports to the Medical Lead.

Primary Focus: The role of the Family Nurse Practitioner is to execute medical care within the scope of an addiction treatment facility, to execute medical protocols that drive long-term sobriety, and to provide medical guidance for department staff. In the Avenues model all medical decisions are made within the context of the behavioral health treatment plan in collaboration with the Clinical Director.

Primary Responsibilities:

Medical Care Protocols

§ Ongoing understanding and maintenance of medical protocols

§ Consult with on-site personnel to evaluate needs based on intake assessment, accuracy of diagnosis, and give direction on individual medical care treatment plan

§ Oversee competent medical monitoring and medication management for the duration of patient’s stay in treatment

Medical Intervention

The primary focus of medical intervention is stabilization and addressing urgent medical needs that will enable a patient to prioritize recovery.

§ Treats patients to address all urgent medical needs, substance use and psychiatric related concerns.

§ Required to be on-call after hours for a predetermined number of days per month.

§ Prioritizes high-acuity patient needs and those with complex co-occurring medical conditions to evaluate and treat urgent needs and provide direction for medical staff.

§ In the extended care phase of treatment outpatient medical services should be recommended in collaboration with clinical team to support program schedule and referrals made post-discharge for non-urgent medical needs and continuing care

§ Provide updated pharmacy orders for LPN to follow up

§ Generate medication, UDS, and lab orders in (KIPU) EMR

§ Respond to positive rapid UDS and review lab confirmation

§ Review and address blood work results

Coordination of Care

§ Communication and collaboration with extended treatment team is critical

§ Escalate all clinical insight and recommendation for follow up by primary therapist.

§ Medical is required to inform clinical and resident staff of issues that require monitoring or potential side effects

§ Collaborate with Utilization Review Team for clarification and to conduct as need Peer Reviews to authorize services

§ Attend morning treatment team meeting as needed

§ Communicate concerns / medical directives to nursing and treatment team

§ Provide support and verbal orders to on-site staff when on-call

Medical Documentation

§ Accurately and comprehensively document medical care and rationale for intervention

§ Document case consults and directives to staff on a case by case basis

§ Complete H&P, IPE, Daily Detox and Weekly RTP patient appointments

Required Education

Graduation from High School or GED/Bachelor’s Degree

Must be able to read, write, speak, and understand the English language

Required Experience

A minimum of 1-year experience in treatment industry preferred.

Licensure/Certification

Current and valid state issued Driver License

Valid CPR certification and First Aid

Job Types: Full-time, Part-time

Pay: $80,768.00 – $157,087.00 per year
