Position Summary Field Medical Care Coordinator | Assistant

At Catalytic Health Partners we are looking for a Medical Assistant/Care Coordinator to help the neediest in our communities. We take care of the highest cost, highest risk patients for health plans. You will complete assessments in member homes and work with a team to meet their social, behavioral, and medical health needs. You will be responsible for completing their social determinants needs assessment, medical history, vital signs and medication reconciliation.

Responsibilities:

  • Interview patients and document basic medical history, vital signs
  • Develop an understanding of the patients’ social and physical needs
  • Organize and schedule appointments-testing to close gaps in care
  • Coordinate and assist patients to advocate and obtain resources
  • Work as a team to meet patient needs and plan for patient success

Skills:

  • Proven experience as a medical assistant or care coordinator
  • Knowledge of medical office management and procedures
  • Excellent time management skills and ability to multi-task
  • Social perceptiveness and service oriented
  • Excellent written and verbal communication skills
  • Degree in medical assistance, certified nursing assistant, behavioral health tech or equivalent
  • Ability to work in a team environment
  • Adaptability to changing scenarios and situations

Job Type: Full-time

Pay: $16.00 – $18.00 per hour

Education:

  • High school or equivalent (Preferred)

Experience:

  • Medical Assistant: 1 year (Preferred)
