GENERAL FUNCTION
: To guide new and existing Salish Cancer Center patients through their Oncology diagnoses from referral to survivorship. To work with the interdisciplinary team to coordinate and execute evidence-based patient care interventions for new and established patients of Salish Cancer Center. To collaborate with leadership to maintain excellence in clinical operations, patient care, and patient experience.


ESSENTIAL JOB FUNCTIONS:

  • Complete comprehensive new patient referral intake to include chart review, benefit investigation, system registration, care coordination with referring and community providers, and internal communication with interdisciplinary team and providers to define patient appointment urgency and outstanding clinical workup tasks to complete prior to new patient consultation.
  • Provide navigation for newly diagnosed patients, including complete workup orders, intake assessments, clinical introductions, and schedule coordination.
  • Collaborates with Integrative Care team to conduct standardized assessments, treatment plans, and clinically indicated interventions for patients with identified barriers to care.
  • Coordinates patient services to allow for care including transportation, interpreters, and other applicable support services.
  • Supports infusion team by conducting triage assessments and documenting plans of care in patient records, routing them to appropriate providers for review and approval as needed.
  • Serves as a resource to infusion nurses for patient education, treatment teaches, and backup clinical duties for patients receiving infusions at Salish Cancer Center.
  • Enact plans of care for patients with provider approval, including prescription fills, diagnostic imaging and testing orders, coordination of treatments, and outside referrals.
  • Work with leadership and the Practice Administrator to develop and maintain processes to improve patient experience, clinical operations, and staff roles within the medical assistant department, and with the other clinical staff.
  • Assist in coordinating patient flow throughout the clinic.
  • Handle in-person, written, and phone requests from patients and caregivers for information and advice.
  • Works with patients, caregivers and agencies to complete and route patient FML, leave, and other forms requests for patient health-related needs.
  • Provide on-site and remote nursing care education to oncology patients and their families and perform other health education activities as assigned.
  • Participate in professional development and community awareness activities.
  • Other duties as assigned.


MINIMUM REQUIREMENTS (EDUCATION AND EXPERIENCE
):

Graduation from an accredited school of nursing, minimum BSN required. Three (3) years of professional nursing experience in oncology required. Must be licensed as a Registered Nurse (RN) in WA State and certified in oncology, or eligible to be certified within six (6) months of hire. BLS certification required. Experience and/or ability to work with electronic health records/practice management systems required. One (1 ) year of supervisory experience preferred.
