Our company holds a high regard to integrity, respect and valuing people. Paradigm is seeking a full time LVN to handle Intake and Scheduling coordination for this growing office. The position is ideal for someone who truly enjoys multitasking and interacting with people. The ability to cultivate positive and long-term relationships with referral sources is integral to success in the position. As an Intake Coordinator/Scheduler, you will facilitate the coordination of referrals and communications for the Paradigm HomeCare office in Plano, TX. Duties will include the following:
Duties will include the following:
– Receiving and documenting incoming phone calls from referral services
– Ensuring referrals are thorough and complete
– Entering all information completely and accurately into patient management system.
– Functioning as facilitator between field clinicians and management team.
– Coordinates the scheduling of all visits for clinicians
– Other duties as assigned
This is a full time office-based position. The position has a competitive base salary, health/dental/life insurance, and 401(k) with employer match.

Requirements:
– Must be a Licensed Vocational Nurse (LVN)
– Must have a minimum of 1-2 years of experience working with the intake and scheduling functions within a licensed home health company
– Demonstrated proficiency in advanced office computer applications
– Excellent customer service skills
– Excellent organizational, communication, and interpersonal skills
