Clinical Manager – Beyond Faith Homecare – Lubbock, TX

General Summary: The Clinical Manager maintains the quality of client care in accordance with the Agencys objectives and policies, through planning, coordination, and implementation and evaluation of the home health services provided. This position also directs and manages the field staff of the Agency while coordinating care with the interdisciplinary team, patient/family and referring Agency.


Patient Population:
All patients assigned to the provider numbers in the employees area of responsibility.


Essential Functions:

  • Plans, coordinates, manages, and evaluates the activities of an assigned team to ensure the delivery of quality assured home health services consistent with the organizations standards of practice, policies and procedures, and in compliance with applicable federal, state and local laws and regulations.
  • Responsible for the supervision, instruction, support, and coaching of assigned field staff in providing quality assured home health services in an organized, cost-efficient manner.
  • Monitors and ensures coordination and implementation of established patient care plans, recertifications, discharge planning, and related activities.
  • Coordinates and conducts regularly scheduled multi-disciplinary case management conferences with assigned staff.
  • Supervises and ensures the timeliness, quality and compliance standards of documentation of assigned staff.
  • Is accountable for and continually evaluates the performance of assigned staff in meeting productivity and qualitative standards, including effective time management.
  • Monitors staffing patterns and needs using performance indicators and maintains appropriate staffing levels to assure timeliness and quality of services provided.
  • Supervises the patient visit scheduling process.
  • Is a resource to staff in matters of policies, procedures, regulations, and coverage issues and directs staff to appropriate resources to resolve problems.
  • Maintains an open line of communication with all levels of staff.
  • Directs clinical staff toward appropriate and available internal and community resources.
  • Performs in-home supervisory visits with assigned staff as needed and in preparation for annual performance evaluation.
  • Performs annual performance evaluations on assigned staff in a timely and constructive manner.
  • Assists in staff orientation and staff development/inservice program in conjunction with the Quality and Clinical Excellence Department
  • Coordinates efforts with other member of the management team to ensure ongoing development and improvement of organization-wide activities, including participation in the Performance Improvement Plan.
  • Performs other duties as assigned, including direct nursing care, if needed.

Experience:

  • License and current registration to practice as a Registered Professional Nurse.
  • BSN or degree in health-related field preferred.
  • Two years satisfactory experience as a Registered Nurse in a certified Home Health Agency.
  • Health care management/supervisory experience preferred.
  • Medicare Experience
  • Excellent communication skills.
  • Excellent interpersonal skills necessary in order to communicate effectively with a wide variety of personnel when identifying and resolving sensitive and complex patient or administrative issues.

Skills:

  • Evidence of progressive leadership responsibilities, including fiscal management is required.
  • Demonstrated knowledge of home health licensure, regulations, and Medicare Conditions of Participation (COP).
  • Excellent communication skills.
  • Excellent interpersonal skills necessary in order to communicate effectively with a wide variety of personnel when identifying and resolving sensitive and complex patient or administrative issues.
  • Nursing skills as defined as generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision-making skills.
  • If required to make visits, Hepatitis profile.
  • Must read, write and comprehend English.

Licensure/Certification:

  • License and current registration as a Registered Nurse.


Environmental/Working Conditions:

  • Prolonged sitting required.
  • Ability to operate office equipment which may include heavy lifting, bending and standing
  • If visiting patients, prolonged standing and walking required, with the ability to lift up to 50lbs and move patients.
  • Ability to handle stressful situations in a calm and courteous manner at all times.
  • Requires working under some stressful conditions to meet deadlines and Agency needs.
  • Performs duties in an office environment during Agency operating hours.
  • Ability to work flexible schedule.
  • Ability to travel locally.
  • Some exposure to unpleasant weather.
  • Available for PRN emergency call at all times either in person or via telecommunications.
  • If required to make patient visits, may work in patients home in various conditions; possible exposure to blood and bodily fluids and infectious diseases.
