Description:
Swedish Medical Center is looking for a Full Time RN Case Manager 10,000 Sign-On Bonus (1.0 FTE / 40.00 hours per week, Day Shift) to work in Case Management at Swedish Cherry Hill in Seattle, WA.
Schedule: 8:30-4pm PST; Rotating schedule where one weekend day (Sat/Sun) would be covered every 4 weeks.
Provides clinical case management functions via a collaborative process that transitions patients to optimal clinical outcomes
while assuring safe, cost-effective care. Demonstrates a daily commitment to the department’s value of teamwork.
In this position you will:
- Performs new admission assessments within designated time frames
- Identifies risk factors and/or strengths of patient/family support system on admission and refers/communicates to appropriate team members. Identifies abuse/neglect and refers appropriately for evaluation and reporting
- Maintains knowledge of particular reimbursement methods and communicates reimbursement information to patients, families and the healthcare team
- Creates and implements transition plans for all patients collaboratively with SW Case Manager
- Promotes health care outcomes in concert with currently accepted clinical practice guidelines and established clinical
- pathways
- Assures patients, families and staff support the appropriate cultural aspects in the delivery of health care. Assures
- availability of interpreter services as appropriate to patient/family needs
Qualifications:
Required qualifications for this position:
- Bachelor’s degree in Nursing Baccalaureate degree (BSN) from an accredited school of nursing required.
- Current Washington State Registered Nurse License
- Case Management Certification required at hire or within 2 years of eligibility
- Three (3) years of current relevant clinical experience
Preferred qualification for this position:
- Hospital-based case management experience.
