CARES Wraparound Case Manager – AK Child & Family – Anchorage, AK

The CARES Wraparound Case Manager (WCM) is responsible for the provision of responsible for care coordination to children and youth and their families by facilitating Family Team Conferencing and the Wraparound process.

Duties include crisis intervention, advocacy, assessment, discharge planning, interdisciplinary teamwork and community liaison.

  • Under the supervision of the CARES Wraparound Coordinator Supervisor, CARES Wraparound Case Manager is responsible for implementing Wraparound process, facilitate Family Team Conferencing and for timely and satisfactory documentation.
  • The CARES Wraparound Case Manager adheres to standards and regulations as set forth by the agency, the Joint Commission, OCS licensing, DBH and Medicaid.
  • The Wraparound Case Manager is expected to abide by all safety rules and regulations of the agency.
  • The responsibilities of the Wraparound Case Manager are carried out according to AK Child & Family policies, the AK Child & Family mission, and the AK Child & Family philosophy of care.
  • Assumes additional duties as assigned.
  • All positions at AK Child & Family may have responsibilities altered during the course of employment due to agency imperatives.

PAY

  • $17.25/hour to $20.00/hour DOE
  • 3% pay increase after successful completion of 6-month Intro Period.
  • Annual raises are based on the annual budget and not guaranteed each year.
