Job Summary
The APS Liaison position is a non-exempt, hourly position, complying with the regulations of the Federal
Labor Standards Act.

Under the supervision of DHHR Policy and Programs Officer, the Aged and Elderly Resource Navigator will provide linkage and referral services to clients over the age of 60 who are experiencing homelessness or who have been recently housed from homelessness throughout the state of West Virginia. The Aged and
Elderly Resource Navigator will assist eligible clients with referrals and connection to needed wraparound services whether in a homeless shelter, a hotel, on the street, or recently housed. The Aged and Elderly
Resource Navigator will work in collaboration with all four Continua of Care (CoCs) and homeless service providers throughout the state of West Virginia to connect clients with the most appropriate wraparound services in addition to the housing services they are currently receiving.

Essential Functions

  • The Aged and Elderly Resource Navigator will work with the WVCEH Coordinated Entry team,

DHHR Policy and Programs Officer, and the Coordinated Entry systems within the Kanawha
Valley Collective (KVC), the Greater Wheeling Coalition for the Homeless (GWCH), and the
Cabell-Huntington-Wayne (CHW) CoCs to identify and develop an ongoing knowledge base of all available mainstream resources throughout West Virginia.

  • The Aged and Elderly Resource Navigator will work collaboratively with the Homeless

Management Information System (HMIS) team across the four CoC’s to identify eligible clients and to reach out to the appropriate homeless service provider.

  • The Aged and Elderly Resource Navigator will build rapport with homeless service providers across the state and develop relationships with key contacts at each homeless service entity in order to have efficient referrals and communication for eligible clients.
  • The Aged and Elderly Resource Navigator will develop a vast knowledge of the homeless system; mental, behavioral, and substance abuse services; and medical services in communities across

WV to link persons over the age of 60 with appropriate wraparound services to assist with housing stability.

  • The Aged and Elderly Resource Navigator will work closely with the WVCEH APS Liaison to connect clients with appropriate Adult Protective Services as needed in each county and region.
  • The Aged and Elderly Resource Navigator will maintain client documentation, including case notes, HMIS entries, and service transactions.
  • The Aged and Elderly Resource Navigator will advocate for the client’s needs and work closely with the DHHR Policy and Programs Office, APS Liaison, and DHHR Bureau for Public Health, to identify barriers.

Core Competencies

  • Knowledge of homeless populations, systems and programs, specifically in-home health services and Adult Protective Services (APS)
  • Knowledge of how wraparound services interrelate and impact homelessness and housing stability
  • A thorough understanding of community resources available to individuals experiencing homelessness and how to access those resources
  • Ability to work independently in the field and as part of a team in various settings as necessary
  • Ability to build relationships with stakeholders
  • Ability to present information thoroughly to a range of stakeholders
  • Ability to demonstrate creative and solution-focused problem-solving skills
  • Experience with Homeless Management Information System (HMIS)

Qualifications and Skills

  • Successful candidate will be a highly motivated self-starter, able to prioritize tasks and manage a complex array of needs.
  • Applicant should have an understanding in all areas related to homelessness, substance abuse and mental health.
  • Ideally, candidates will have prior experience with people experiencing homelessness.
  • Must have good problem-solving skills, communication and conflict resolution skills, and be proficient with computers.
  • Prior experience using the Homeless Management Information System (HMIS) is a plus but not required.
  • Applicant will need an understanding of community resources and the wherewithal to navigate complex eligibility requirements for community resources.
  • Professional, mature demeanor with the ability to work independently is required.
  • The successful candidate will have reliable transportation and must be willing to travel occasionally throughout the designated region and to State office in Bridgeport, WV.
  • Although a Bachelor’s Degree is not required, it is preferred. Past experience working in a case management role will be taken into consideration.
