Job Summary
The APS Liaison position is a non-exempt, hourly position, complying with the regulations of the Federal
Labor Standards Act.

Under the supervision of DHHR Policy and Programs Officer, the APS Liaison will work closely with local
DHHR offices across West Virginia to garner a deep understanding of Adult Protective Services and operations at each county office. The APS Liaison will develop relationships with homeless service providers across West Virginia to bridge services with APS for efficient care to the most vulnerable individuals.

Essential Functions

  • The APS Liaison will work with the WVCEH team, DHHR Policy and Programs Officer, the Kanawha Valley Collective (KVC), the Greater Wheeling Coalition for the Homeless (GWCH), and the Cabell-Huntington-Wayne (CHW) CoCs to develop relationships with providers regarding APS referrals for clients.
  • The APS Liaison will develop relationships with key contacts at each APS office across West Virginia in order to gather information and work through challenging situations.
  • The APS Liaison will develop a vast knowledge of Adult Protective Services, state code, and operations at each county office.
  • The APS Liaison will work closely with APS staff to identify and bolster collaboration with medical professionals and other community providers involved with client services
  • The APS Liaison will advocate with Adult Protective Services for the clients’ needs and work closely with the DHHR Policy and Programs Office and DHHR Bureau for Public Health to identify barriers.
  • The APS Liaison will maintain appropriate documentation in HMIS.

  • A knowledge of Adult Protective Services, as well as the homeless population systems and programs
  • Ability to work independently in the field and as part of a team in various settings as necessary

  • Ability to present information thoroughly to a range of stakeholders
  • Ability to demonstrate creative and solution-focused problem-solving skills
  • Experience with Homeless Management Information System (HMIS)

Qualifications and Skills

  • Successful candidate will be a highly motivated self-starter, able to prioritize tasks and manage a complex array of needs.
  • Applicant should have an understanding in all areas related to homelessness, substance abuse and mental health.
  • Ideally, candidates will have prior experience with people experiencing homelessness.
  • Must have good problem-solving skills, communication and conflict resolution skills, and be proficient with computers.
  • Prior experience using the Homeless Management Information System (HMIS) is a plus but not required.
  • Applicant will need an understanding of community resources and the wherewithal to navigate complex eligibility requirements for community resources.
  • Professional, mature demeanor with the ability to work independently is required.
  • The successful candidate will have reliable transportation and must be willing to travel occasionally throughout the designated region and to State office in Bridgeport, WV.
  • Although a Bachelor’s Degree is not required, it is preferred. Past experience working in a case management role will be taken into consideration.
