Assess nutritional status.Provides nutrition ed & counseling services to individuals & groups in a variety of settings:Discuss identified nutritional risk factors,Develop plan with participant for improvement of dietary patterns & practices, Referral for additional services, Work with internal & external partners for services, Develops other ed materials as needed. Recommends dietary changes & assigns supplemental food packages to improve nutritional status.
Registered dietitian or baccalaureate or master’s degree in dietetics, nutrion, public health nutrition or related field; OR registered nurse; OR home economics (must have a min of 20 hrs in food & nutrition course if economics degree).Valid IL driver’s license & insurance.
