JOB SUMMARY
Responsible for safety of residents under their supervision. Manages compliance with Federal and State regulations, Center policies and procedures, and standard of practice with regard to resident care. Validates nursing staff functions within the scope of their practice and complies with regulations, policies, and procedures. Performs other duties as assigned by supervisor.
ESSENTIAL FUNCTIONS:
Interprets existing policies and procedures to other nursing personnel and nursing assistants, restorative nursing assistants, residents, families, and physicians.
Assesses resident needs and initiates development of individualized resident care plans. Utilizing the nursing process, Minimum Data Set (MDS), Care Area Assessment (CAA), and care plan:
Reviews and updates resident care plans and Certified Nursing Assistant (CNA) flow sheets as necessary and as assigned in compliance with regulations.
Conducts interdisciplinary resident care conferences as scheduled.
Supervises and evaluates direct resident care provided within assigned unit and initiates corrective action as necessary.
Evaluates resident care as related to individualize resident needs, family involvement, and the physician’s plan of care for the resident.
Obtains report from nurse and records sufficient information to implement appropriate follow-up action in relation to the residents’ treatment needs.
Provides nurse on duty with concise, accurate report and takes follow-up action as necessary.
Identifies special nursing problems and emergency situations quickly, and initiates appropriate interventions for individual residents.
Administers treatments per physician order, is responsible to document per Center procedures.
Supervises and evaluates the implementation of Resident Bill of Rights.
Documents resident care provided and resident’s response or lack of response to treatment provided.
Assesses and reports changes in resident’s condition, including development of pressure ulcers, to physician, the DNS and responsible party, and takes recommendations for nursing action to be implemented.
Assesses resident responses to medication and treatment, and makes appropriate recommendations for nursing action to be implemented.
Assesses on a weekly basis via resident rounds the condition of existing pressure ulcer by stage, size (measurements), sites depth, color, drainage, and odor. Reports problems to the DNS; takes necessary follow-up action.
Assists in the admission, transfer, and discharge of residents as necessary and assigned by DNS. Completes admission, transfer, and discharge documentation.
Renders immediate assistance to residents in need.
Supervises the timely response of call-lights within assigned unit.
Listens to resident, family, or physician complaints and initiates corrective action and/or reports special problems to the DNS.
Follows up on physician orders, lab, and x-rays.
Observes resident, records significant conditions and reactions, and notifies family (responsible party) and physician of resident’s condition, and reaction to drugs, treatment, and significant incidents. Documents it according to Center policy and Standards of Practices.
Schedules resident appointments and notifies transportation for transport arrangements.
Rounds with physicians as needed and process physician orders.
KNOWLEDGE, SKILLS, AND ABILITIES:
Licensed as a Registered Nurse in the state of Nevada.
One-year previous experience in hospital, skilled nursing center, or other related medical facility preferred.
Training in rehabilitative and restorative nursing practices preferred.
Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
Requires mental maturity to deal with fast-paced work environment, with occasionally combative residents.
