RN Nursing Supervisor all shift – Oceanview Nursing & Rehabilitation Center – Queens, NY

PRIMARY OBJECTIVES

1. The Unit Coordinator is responsible for assessing, planning, implementing, evaluating, supervising and completing nursing care delivered to all residents on the unit, and for coordinating the interdisciplinary processes shared by other departments.

2. The Unit Coordinator is responsible for overall management of the unit for a 24-hour period.

3. The Unit Coordinator is directly responsible for the supervision of:

LPNs

Aides/Orderlies

Students/ Volunteers (when present)

RESPONSIBILITIES

1. Organize nursing care for all the residents on the unit.

a) Verify L.P.N completes and/or reviews assignments daily with special attention to:

Continuity of care.

Time for classes, meetings.

Monitoring prompt arrival/leaving of staff on unit.

Equitable distribution of workload.

Directs staff assignment according to assessed priorities.

b) Maintains up-to-date care plan (as assigned) by:
Making rounds with physician and conversing with residents.

Keeping staff members informed of new or revised physician’s orders and making necessary changes in care plans.

Observing resident needs.

Reviewing nurses’ notes from other shifts.

Reviewing therapy reports, tests and consultations.

Conducting team conference Comprehensive care planning, CCP) meetings to discuss resident care and determining if set goals are achieved.

1. Delegates responsibility and discusses resident assignments with staff and gives necessary instruction regarding specific details in relation to assignment.

Assigns L.P.N. to up date C.N.A. accountability records, on at least a monthly basis.

Monitors that L.P.N. documents accurately the resident’s conditions and needs.

Checks on a monthly basis that C.N.A. accountability is being signed appropriately.

2. Responsible for necessary follow-up with other shifts regarding resident care, documentation and other related matters.

Communicates with other shifts, all tours concerning information essential for continuity of care.

Collaborates with team members to evaluate solutions that improve care/ services to residents

3. Continually observes, supervises and evaluates quality, quantity and performance given by staff members (including agency nursing staff).

Identifies educational needs of staff

Provides incidental teaching of nursing staff

Collaborates with infection control/ in-service educator and trainer in giving orientation/ in-service instruction to nursing personnel.

Assists in investigation/ resolution of complaints related to nursing practices.

6. Interprets philosophy, objectives and policies of the facility to staff, residents, relatives, and visitors.

7. Ascertain that all-unsafe conditions and environmental conditions are reported and an appropriate repair request made out.

8. Coordinates (and verifies completion of investigative forms prior to submission to D.N.S.) investigations when related to safety of residents and plans of corrections (e.g. – Accident/ Incidents).

Ensures narcotics and controlled drugs are accurately recorded according to law.

9. Completes the Medicaid Minimum Data Sets (MDS) to reflect nursing needs of the residents, as assigned by MDS Coordinator/ or designee.

Checks accuracy of nursing sections (and quality indicators, as assigned).

Validates that resident conditions/ abilities in MDS assessments match documentation in medical record, care plans, C.N.A. accountability assignment, physician orders and treatment/ medication records.

Confirms that entire interdisciplinary team completes and signs their sections within the time frame (assisted by MDS coordinator).

10. Conducts staff meetings to clarify matters such as policies, interpersonal relationships and lines of authority, and summarize in the minutes pertinent results.

11. Conducts unit rounds (at least twice, in beginning/ end of every shift) to assure safe environment, resident care delivery, resident satisfaction and staff offers a cohesive team work.

C.N.A. is providing care/ service (turning residents at high skin risk every two hours)

L.P.N. overseeing C.N.A. work flow, breaks and documentation.

12. Conducts audits to verify residents receive treatment, medication, prescribed diet, rehabilitative nursing care, proper skin preventive care, appropriate positioning to prevent deformity, and kept clean, well groomed and physically/ally motivated at their optimal levels.

13. Start intravenous fluids upon physician orders.

Initiates, monitors and documents IV medications per physician orders.

Cares for central lines as per physician orders.

Monitors for central line infections signs/ symptoms and reports to physician immediately.

8. Acts as liaison between unit and other departments.

9. Teaches and/or assists in orientation of new personnel.

Assigns appropriate mentors to new orientee.

Assesses safety of practices and competency of new hires (before probation ends).

Skills new employees to assure safe practices.

15. Evaluates annually, or as necessary, performance of nursing staff and makes recommendations for hiring of new staff and promotions.

16. Counsels and disciplines nursing staff:
a) May recommend disciplinary action such as but not limited to: counseling, suspension, discharge, reassignment, transfer, etc.

b) Collects and analyses data for purpose of determining validity for counseling, complaints, and/or grievances.

17. May immediately suspend and/or require staff to punch out and leave the facility for such unacceptable actions as- drunkenness, fighting, resident abuse, refusing an assignment, insubordination, etc.

18. May recognize quality performance of staff via written recommendations for such employees.

19. Attends and participates in in-service educational programs and management meetings, as necessary.

20. Assists in nursing research related to improving resident care.

21. Identifies and studies nursing problems and assists in their solution.

22. Performs all duties of staff nurse, as necessary.

23. Takes call every other weekend.

24. Performs other related duties as assigned by Director of Nursing Service.

QUALIFICATIONS

1. Licensed and currently registered as a professional nurse in New York State.

2. Demonstrated competence as a nursing practitioner, including management and leadership abilities.

3. Trained in MDS, CCP and leadership skills.

4. Trained to start IV access (preferred).

5. Participates annually in continuing nursing education.

6. CPR certification preferred.

7. Attendance in infection control training preferred.

8. Demonstrated commitment to continuing professional development.

9. Has experience in working as a nursing supervisor in a nursing facility (preferred).

Knowledge, Skills and Special Abilities

1. Comprehensive general nursing background.

2. B.S.N. preferred

3. Ability to interpret policies, set standards, using sound judgment and organize effectively.

4. Minimum of one year experience in supervision in a nursing facility.

5. Strong leadership and interpersonal skills.
