Responsible, as part of a cross-functional team, for designing and implementing projects and programs across various functional areas and business units related to pharmacotherapy optimization for value-based contracted patients. Provides subject matter and clinical expertise in various areas (including population health initiatives and Case Management), creates treatment plans for chronic diseases (i.e. COPD, DM, CHF), works to close quality gaps (i.e. medication adherence, statin use), and delivers drug information. Makes presentations to local and national committees and management to establish project deliverables and expected results. Collaborates with stakeholders across UHC, Optum, Care Delivery Organizations etc. to ensure clinical affordability projects are aligned with core strategies and goals.

Primary Responsibilities:

  • Complete discharge medication reconciliation and provide recommendations to providers and counseling to patients transitioning from hospitals, SNFs, institutions or other high-risk patients
  • Performs comprehensive medication review to include all prescription and non-prescription drugs, herbals, and supplements to identify medication related problems:
    • Reconciles medication lists to identify and resolve discrepancies
    • Evaluates safety, efficacy, and appropriateness of regimen against patients’ medical conditions for opportunities to promote individualized therapy optimization
    • Utilizes MTM platforms where feasible
  • Synthesizes detailed clinical evidence, safety profiles (i.e. QOL), utilization metrics, and real-world data related to medications & medical conditions to identify opportunities, establish care pathways/clinical algorithms, and implement population-based interventional strategies
  • Develops, implements, and supports pharmacy & medical strategies and programs to achieve key strategic and business goals
  • Partners with executive leaders in the development of organizational objectives and goals to identify/define performance planning/measures, programs, and initiatives related to emerging trends and growth opportunities
  • Collaborates with functionally diverse teams (i.e. MD, RN, PharmD, LCSW, PM) to achieve desired end-results and develop innovative solutions for complex problems
  • Provides medication optimization and quality improvement interventions for patients to improve medication-related Medicare Part D 5 Star and HEDIS measures
  • Facilitates functional and segment teams through project implementations with competing agendas and priorities
  • Produces appealing visualizations leveraging multiple internal and external data sources (quantitative and qualitative) to draw insightful conclusions for decisions makers
  • Solves unique and complex problems with broad impact on the business
  • Provides subject matter expertise and operational support to internal business partners
  • Partners with senior leadership to adopt new ideas, products, and/or approaches that may have segment-wide impact
  • Performs all other related duties as assigned

Required Qualifications:

  • Graduation from an accredited College/School of Pharmacy with a B.S. with 10 years of clinical experience or a Doctorate of Pharmacy (PharmD)
  • Active, unrestricted Pharmacist License in WA
  • At least one year of Post-Graduate/Residency training or equivalent 3 years of practice.
  • At least two years of medication therapy management and medication reconciliation experience.
  • Demonstrated leadership & communication skills in developing content and presenting to various committees/management to drive consensus recommendations and clinical decisions
  • Must have strong prioritization, attention to detail, effective multi-tasking, verbal/written communication and organizational skills.
  • Must have the ability to create an effective team environment, build strong relationships.
  • Must be strong in solving problems in a mixed-matrix environment and communicating effectively at all levels of an organization.
  • Proficiency with Microsoft Office Tools and other technical applications (i.e. EMRs)
  • Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance

Preferred Qualifications:

  • Board Certification in pharmacotherapy, geriatrics, or ambulatory care
  • Managed care experience
  • Experience with management of HEDIS and CMS STAR measures
  • Epic EMR experience
