We’re seeking social workers and/or mental health professionals to join our team as the Clinical Services Specialist – PATH Program Coordinator . In this position, you are a critical member of the Multnomah County Behavioral Health Division Promoting Access to Hope (PATH) Program. This is a collaborative project involving local police, prosecutors, public defenders, corrections officers, social service providers and community organizations. You will provide critical clinical assessment, analysis, treatment services, specific engagement, care coordination, and case management to individuals suspected of low-level drug offenses, houseless individuals, at risk of becoming houseless, and individuals suffering from substance use disorder (SUD). The position requires advanced and demonstrated knowledge and experience working with individuals struggling with houseless issues, substance use disorder and legal involvement through a trauma informed lens.

Direct Services

  • Provide direct assessment, analysis and intervention services to a caseload of approximately 15-20 individuals.

  • Provide outreach, engagement, supportive services as well as access to services for participants through intensive care coordination and case management activities.

  • Coordinate and collaborate with Substance Use Disorder (SUD) service providers, housing providers, primary care, social service agencies and other community organizations.

  • Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.

  • Meet with clients ‘where they’re at’, and screen participants for the severity SUD, housing status and determine needs for other services, e.g., medical, SUD services, housing etc.

  • Assist participants in gaining access to a variety of social services programs including SUD treatment, e.g., supportive housing, peer support, Alcoholics Anonymous/Narcotics Anonymous (AA/NA), medical services, etc.

  • Maintain collaborative relationships with stakeholders including Multnomah County Behavioral Health, Portland Police Department, Multnomah County Sheriff Office, District Attorney (DA), Department of Community Justice, Joint Office of Homeless Services (JOHS).

  • Develop and maintain collaborative relationships with community partners who have deep relationships with the community. This may include but not limited to civic, faith-based organizations, services providers, and governmental groups.

  • Maintain working knowledge of the behavioral health system with particular emphasis on substance use disorder services and resources.

Community Relationships

  • Maintain partnerships with contracted agencies, health care providers, and other social service providers as contact points with Behavioral Health Division (BHD).

  • Build mutually beneficial relationships with providers and community partners especially those who offer culturally specific services to research and document developing needs within various communities served.

  • Collaborate with other Health Department teams, Department Community Justice, Multnomah Sheriff’s Office, DA’s office, JOHS, community partners and external agencies to develop service paths for clients, understand community needs, and to identify barriers to care.

  • Attend community outreach meetings including but not limited to Homeless Alcohol and Drug Intervention Network (HADIN), Women’s Networking Breakfast, Service Coordination Team Meeting and other monthly community outreach meetings focused on the community that is being served.

  • Act as contact for multi-disciplinary teams to consult and/or intervene in highly complex and difficult cases for diverse clients and settings in the community with community partners, internal care teams, etc.

  • Act as a response point to emergency needs or investigation requests of individual clients, service providers, etc.

Documentation

  • Maintain up to date notes on members served within the assigned Electronic Health Record system to track care, service plans per member, next steps or missing resources needed in care plan, etc.

  • Complete notes as needed for billing and grant funding for any services provided and coordination of care between and with member organizations.

  • Enter, control, maintain and report out as appropriate investigations conducted, clients with urgent or specialized needs that may not fit into standard practice or system of care, coordinate findings with team members as part of investigation team or as internal resource, potential violations of policies or processes from internal and external partners, and work with program leadership on management of urgent issues for reporting requirements.

  • Respect and appreciation for ethnic and cultural diversity.

  • Ability to apply an equity, intersectional and anti-racist lens.

  • Ability to provide information/referral to community mental health providers.

  • Crisis intervention, including risk assessment of clients who may be involved in potentially life threatening situations.

  • Advanced skills in managing sensitive information and ability to support the community.

  • Knowledge of cultures, customs and social experiences of communities experiencing marginalization and barriers to opportunity.

  • Ability to exercise good judgment, diplomacy, and tact in dealing with sensitive, complex, confidential information and situations, in addition to acting professionally, and emphatically, with individuals, internal/external partners and communities.

Required Minimum Qualifications/ Transferable Skills*:

Education/Experience:

  • Equivalent to an Associates degree in social services, psychology, sociology, counseling or related field and four (4) years professional work experience as a counselor, advocate, or social worker ; OR,

  • Equivalent to a Bachelor’s degree in counseling, social services, psychology, sociology, social work or a related field and two (2) years of professional work experience as a counselor, Mental Health treatment professional, social worker, or therapist; AND,
