The MDS Nurse is a nurse who has developed assessment and clinical skills through experience and education. Whether you work directly with patients or use your skills to support those who provide direct care, you will be part of a team that treats residents’ needs as their own. You should have the ability to coordinate and organize nursing care related to the resident assessment, as well as conform to Medicaid/Medicare guidelines.
As an MDS Coordinator, you are to:
- Assist the interdisciplinary team (IDT) with ensuring that documentation meets the Federal, State, and Certification guidelines
- Coordinate, gather data, and complete Resident Assessment Instrument in an accurate and timely manner
- Attend and participate in resident care plan meetings
Qualifications:
- Graduate from an approved Nursing Program
- Experience in Long Term Care setting preferred
- Current New York State Nursing License (RN Preferred, LPN license with experience may apply)
- MDS Certification (i.e RAC-CT) is preferred but not required
- Certified PRI preferred but not required
