- Responds to patient and family calls by making decisions utilizing independent judgment and works effectively in atypical, urgent and crisis situations * Responsible for assessment, planning, implementation, evaluation and revision of nursing needs for a patient/family caseload * Participates in patient care meetings and staff meetings as requested * Collaborates with physicians, Medical Director and other staff regarding changing conditions. * Supervises caregivers, volunteers, Home Health Aides, Homemakers and other nursing staff on the delivery of patient care plans * Performs administrative functions relating to clinical records, Plan of Care, and mandatory abuse reporting requirements * Provides consultation and support to patients/families regarding Hospice services, Medicare Hospice benefits, and during emotionally stressful times. * Participates in community education, volunteer and Hospice activities and in-service as requested. * May be assigned to any workplace setting based on LRB needs including office, Patient Home * Provides triage for home visits, hospitalization, or physician office visits and nursing advice to determine immediate level of care required * Responsible for notification to attending physician, DME, pharmacy and any other entity of a LRB IN HOME HOSCPECE CARE patient’s death * Assists with managing/processing of faxes/orders as assigned * Supports Intake by responding to phones calls, processing referrals, determining, with hospice Medical Director, eligibility and initiating admission process * Participates in Quality Assurance Performance Improvement as assigned * Is knowledgeable of agency guidelines regarding patient referrals, physician/discharge planner relationships, and consumer complaints * Serves as a field nurse as needed during staffing shortages and vacations * Meets the criteria of the per diem agreement for availability each month * Able to function in role of visit nurse, triage nurse, and admission nurse based on unit’s need
