General Summary

Under the direction of the Executive Director, the Diabetes Program Manager RN CDE will be responsible for development of educational consultation and group programing across all NSPG practices, focusing on guiding individuals living with Diabetes improving overall wellness and clinical outcomes. The program manager will act as a clinical liaison between endocrinologist, PCP, office RN and various administrative departments. The Program Manager is responsible for conducting health assessments and diabetes education according to established American Diabetes Association curriculum as well mantaining NSPG’s accrediatation status and marketing to larger community. The program manager must be organized, detial oriented, maintain strong up to date clinical skills and have excellent interpersonal skills.

Responsibilities

  • Conducts individual health assessments on diabetes patients.
  • Conducts group and individual diabetes education sessions and class instruction for newly diagnosed or high–risk patients and their family members according to established curriculum.
  • Responsible for the development of education consultation and group programming
  • Responsible for maintaining Diabetes Accreditation status
  • Quality Coordinator: maintaining NSPG AADE accreditation, updating curriculum regularly to reflect the lastest evidence based treatment.
  • Reports on the annual CQI project to NSPG and AADE
  • Continual research and maintence of up to date information on diabetes, technology, and devices.
  • Responsible for quartely Quality improvement.
  • Responsible to Market program to providers, Pateints and the Public via scoail media, lectures, communicty education, gateway and social media.
  • Regularly identifies organization wide – clinical education / traning needs.
  • Participates in the trainnig of new RN’s and Ma’s as it related to chronic diseease and Diabetes.
  • Develops and implements standard practice based compencies for all new medical devices and assiti with set up for providers
  • Communicates clinical education program information , updates and other pertinent inforation to the internal and external customers.
  • Works with patients and family members from diverse ethnic backgrounds and wide age spans.
  • Responsible for understanding all CPT changes related to DM education.
  • Assists in maintaining a safe and clean environment in accordance with the OSHA, Infection Control and Safety Policies.
  • Participates in quality improvement activities.
  • Adheres to hospital wide standards and department specific standards for service excellence, including, but not limited to: identification of and response to customer needs, appropriate communication, protection of patient privacy and confidentiality, etc.
  • Assumes responsibility for continuing professional education and development.
  • Maintains all required licensure and certifications.
  • Keeps up-to-date on best practices for Profession.
  • Maintains competence through regular participation in service education programs and other educational opportunities.
  • Assists with ensuring the safe operation and compliance with State, Federal and JCAHO requirements, regulations and standards.
  • Maintains NSPG program certification and submits timing quartely reports and improvement activities.
  • Display independence of thought and action in daily task, problem solving and initiation of analyses
  • Demonstrates administative , orgainzatonal support through commiteee attendance and participation in the following :Managers meeting, Diabetic boot camp, NSPG quality Council and Nursing Council

Experience

Required: Experienced RN with a minimum of 3 years of working with Type 1 and 2 Diabetic Pateints and understands complex uses of all insulins, CGMs and glucometer education

Education

Required: Completion of an accredited Nursing program

Preferred: BSN

Licensure

Required: CDE in addition to Licensed to practice as an RN or NP in the state of Massachusetts required. CDE may be preferred or required.*Per ADA/reimbursement regulations, the department must have at least one Certified Diabetes Educator on staff to conduct the Diabetes Group Education. The CDE requirement may be fulfilled by either the RN role or the Dietician role.
