Continuum Rehab Group is currently seeking talented COTA’s to join our PRN team at our SNF in Greenville, TX!


Position Description

Responsible for assisting the Occupational Therapist and/or supervisor in the overall day to day operations of occupational therapy services of the center. Provides assistance in coordination with other divisions, departments and nursing staff to ensure that quality care is provided and to support the goals and objectives of the company. Promotes wellness through the assistance of completion of patient assessment, care planning, direct treatment and consultation to a general rehab caseload.


Principal Responsibilities

  • Meets the resident’s goals and needs and provides quality care by assisting in the assessing and interpreting evaluations and test results; determining occupational therapy treatment plans in consultation with physicians or by prescription.
  • Maintain productivity as set forth by the Director of Rehabilitation and/or Regional Director.
  • Helps residents develop or regain physical or mental functioning or adjust to disabilities by assisting in the implementation of programs involving manual arts and crafts, practice in functional, prevocational, vocational, and homemaking skills, activities of daily living, and sensorimotor, educational, recreational, and social activities; directing aides, technicians, and assistants.
  • Promotes maximum independence by assisting in the selecting and construction of therapies according to individual’s physical capacity, intelligence level, and interest.
  • Assist therapist in supporting residents return to community; determining potential resident difficulties; helping resident and caregivers understand necessary physical accommodations.
  • Assist in the implementation of individualized therapy programs designed to restore, reinforce and enhance performance.
  • Identify need for and make referral to other disciplines to address the comprehensive needs of the patient.
  • Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology.
  • Maintain temporary licensees, support other students and support personnel in accordance with state licensure requirements and professional standards
  • Assist in the screens and evaluations of patients and develops an appropriate plan of care.
  • With supervision, provides direct patient care in accordance with physician’s orders and under an established plan of care. Provides clinical documentation on a timely basis.
