Job Description

The Patient Advocates team comes together each and every day, to make a difference in our members lives. The healthcare system is large and complex. Best understood by the professionals working in it day in and day out. The healthcare system for our members can be intimidating and overwhelming. This is especially so when someone has a serious diagnosis and is needing complicated treatment. Our role at Patient Advocates is to walk hand in hand with our members as they progress on their journey. We answer questions, provide information and support to our members aimed at making the journey easier.

Patient advocates is seeking professional team players with a passion for helping others. You will support members navigating the complexity of the healthcare system. Be a part of our team:

  • Answer inbound calls guiding members to the appropriate care and support.
  • Call members offering support and monitoring member needs.
  • Serve as a team member supporting the patient across the continuum of care: Home, Outpatient, Acute Care, Skilled Nursing Facility, Rehabilitation, Home Health and Hospice.
  • Educate and empower the patient to advocate for self.
  • Support patients and families in coordinating care.
  • Support referrals of member to high quality providers.
  • Communicate with participant/ patient, families, health care providers and other resources involved in the participants care to obtain complete and accurate information.
  • Support the team by providing administrative assistance.
  • Schedule consults with Centers of Excellence.
  • Document all calls in patient’s chart accurately and objectively, applying the principles of the nursing process.
  • Coordinate and refer according to Value Based Pricing plan language. Assist in redirecting to lower cost facilities.
  • Other duties as assigned

Minimum Qualifications and Skills Needed

  • Experience managing phones – inbound and outbound calls
  • Strong written and verbal communication skills
  • Ability to plan, coordinate and prioritize
  • Ability to perform critical analysis and problem solve
  • Demonstrate ability to advocate for patients
  • Knowledge of EOB’s, CPT, HCPC, ICD codes, UB’s and authorization and referrals helpful
