Position Summary:

The Wellness Director will develop, implement and manage policies procedures, train nursing personnel, ensuring compliance with applicable regulations and standards.


Required Experience for Wellness Director:

  • Graduation from an accredited nursing school or equivalent health care or social work-related curriculum required.
  • BS desirable.

  • RN Requirement
  • Recent supervisory experience in skilled nursing.
  • Computer experience with work processing, spreadsheets and data base software.
  • Must have applicable federal/state certification or licensure.
  • Must have excellent oral and written communication skills.
  • Must possess leadership abilities, initiative and interpersonal relations.
  • Current CPR and First Aid credentials.
  • Must have strong computer skills.
  • Must be familiar with an integrated clinical/accounts receivable system, payroll system, and Internet.


Primary Responsibilities Wellness Director:

  • Achieve or exceed budgeted NOI.
  • Maintain current and accurate Spend-down Worksheets.
  • Enforce and follow purchasing policies and procedures.
  • Ensure proper billing and contracting activities are in place for resident services.
  • Coordinate with Assisted Living and Memory Care Program Directors to achieve maximum staff economies and cross training, if applicable.
  • Maintain program standards in excess of license regulations.
  • Organize, monitor and direct an ongoing activity-based program consistent with our company philosophy. Assure associate training for activity-based program
  • Assist in care-giving as needed.
  • Assist in developing, implementing and periodically updating the written procedures for discharge planning.
  • Ensure that a stock level of medications, medical supplies, equipment, etc., is always maintained on premises to adequately meet the needs of the residents.
  • Plan, organize, develop, and direct the overall operation of the nursing department in accordance with current federal, state, local, and Common Sail Investment Group standards, guidelines, and regulations.
  • Attend all management meetings as requested.
  • Develop, maintain, and periodically update written policies and procedures for your department.
  • Ensure that associate work schedules for your department are legible and posted at least two weeks in advance.
  • Maintain responsibility for the planning, organization, and monitoring of nursing department function.
  • Make written and oral reports/recommendations to the Administrator as necessary concerning the operation of your department.
  • Perform administrative duties such as completing medical forms, reports, charting, etc., as necessary.
  • Serve on various committees as necessary or as requested by the Administrator.
  • Assist in developing the agenda for care planning meetings as necessary.
  • Evaluate and implement recommendations from established committees as they may pertain to nursing services.
  • Make daily rounds of the nursing department to ensure that all nursing associates are performing their work assignments in accordance with acceptable nursing standards.
  • Provide complaint/grievance reports to the Administrator as they arise.
  • Encourage attending physicians to record and sign progress notes, physicians’ orders, etc., on a timely basis and in accordance with current regulations.
  • Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.
  • Assist to develop the annual marketing plan for the program.
  • Actively participate in outreach in accordance with the marketing plan.
  • Take advantage of opportunities to publish news items relating to Skilled Nursing.
  • Participate in personal visits for prospective residents and/or families.
  • Actively participate as a member of the Safety Committee and support the safety program regularly throughout the year.
  • Actively participate as a member of the Infection Control Committee.
  • Ensure that you and all associates in your department have been properly trained and are knowledgeable in the use of the Risk Management, Injury and Illness Prevention manual, MSDS books, and Emergency Preparedness manual.
  • Ensure that all personnel performing tasks that involve potential exposure to blood, blood-borne pathogens, or other bodily fluids or hazardous materials have participated in appropriate in-services or other training prior to performing such tasks.
  • Ensure that nursing associates follow established infection control procedures when isolation precautions become necessary.
  • Ensure that all nursing associates wear and/or use safety equipment and supplies whenever needed.
  • Report and investigate all allegations of resident abuse and/or misappropriation of resident property.
  • Conduct and document team meetings for your department at least bi-weekly.
  • Provide job-specific orientation and training to all associates in your department before they begin their first shift.
  • Ensure that all associates in your department attend general orientation within two weeks of hire or as required by law, whichever is sooner.
  • Provide customer service training to all associates in your department on an ongoing basis.
  • Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your current license(s).
  • Ensure that the HR Director approves every involuntary separation in your department.
  • Meet or exceed the Associate Satisfaction indicator in your Personal Development Plans.
  • Respond to associate requests in a timely and friendly manner.
  • Maintain confidentiality with all associate information.
  • Recruit, hire, train, acknowledge, evaluate, and discipline associates for your department per company policy.
  • Monitor associate time worked.
  • Maintain appropriate reference materials for nursing staff.
  • Ensure compliance with all company policies and procedures.
  • Ensure that your department has fully implemented the company compliance plan.
  • Assist in developing and implementing appropriate plans of action to correct identified deficiencies.
  • Review and respond to comment cards for your department as requested by the Administrator.
  • Respond to resident and family requests in a timely and friendly manner.
  • Meet or exceed the Resident Satisfaction indicator in your Personal Development Plans.
  • Maintain confidentiality with all resident information.
  • Ensure associate documentation of service provided via medical record system and federally mandated MDS system.
  • Provide oversight and guidance to care planning team.
  • Review and revise care plans and assessments as necessary, at least quarterly.
  • Assure adequate staffing is in place to provide for residents’ total regimen of care.
  • Ensure availability of supplies.
  • Meet with consultants to review recommendations and take appropriate action.
  • Develop methods for coordination of nursing services with other resident services to ensure the continuity of the residents’ total regimen of care.
  • Provide the Administrator with information relative to the nursing needs of residents and the nursing department’s ability to meet those needs.
  • Inform nursing associates of new admissions, their expected time of arrival, room assignment, etc.
  • Ensure that rooms are ready for new admissions.
  • Greet newly admitted residents upon admission and escort them to their room as necessary.
  • Schedule physician visits as necessary.


General Working Conditions:

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires employee to effectively present information in one-on-one a small group situation with residents, guests, and other associates. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. Employee must be able to read, write, and speak fluent English. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
