Licensed Practical Nurses who choose to focus on home healthcare are a special group of people that can provide direct patient care while also educating caregivers and actively participating on the patient’s care team. This is a level of nursing that must be able to accurately assess needs, provide appropriate interventions, and revise care as needed under the supervision of a registered nurse. WHAT YOU WILL DO

  • Implement nursing plan of care at the direction of the registered nurse.

  • Provide home-based patient care based on physician orders and the established nursing plan of care.

  • Report any change in patient status to the Home Health RN and document as appropriate.

  • Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient’s treatment needs in order to achieve defined goals for improved patient outcomes.

  • Conduct patient psychosocial assessments to explore the physical, psychological and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs in order to inform care team.

  • Contribute to developing discharge plans, identifying the post-discharge needs of the patient and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.

  • Document patient care information and ensure timely and accurate completion of patient care records in accordance with policies and procedures.

  • Participate in the education of patients and their caregivers about extended care facilities, home health agencies, and other resources as appropriate to transition patients along the continuum of care.

WHAT IT TAKES TO SUCCEED

  • Prior Acute care or Long Term Care experience.

  • Prior experience in Home Health.

  • Educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.

  • Develop effective relationships with patients, hospital departments, and other external parties.

  • Effectively communicate verbally and in writing.

  • Handle multiple responsibilities simultaneously.

  • Understanding of Home Health regulations.

  • Interpret test results and formulate reports.

  • Knowledge of principles of quality patient care.
