SUMMARY

The Santa Monica C3 Case Manager will perform a variety of tasks to assist the most vulnerable and chronically homeless individuals and is responsible for identifying, engaging, and providing support to clients, including providing aftercare and follow up to clients. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of Santa Monica C3 is to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The Case Manager provides information and support for program members and assists members in linking to a variety of resources and services as needed. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Identify, engage, and provide support to clients in Santa Monica C3 program
  • Provide case management and aftercare/follow-up services to clients during their journey through recovery from homelessness
  • Report all statistics and updates for reporting purposes
  • Provide initial contact, needs assessment, information, education, and referrals to the homeless
  • Provide case management, basic services, and advocacy for clients
  • Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.)
  • Develop effective, trusting relationships with clients that facilitate provision of services
  • Work in collaboration with C3 team members and outside agency staff to connect clients to appropriate services
  • Maintain and record accurate electronic records, client files, and statistical information
  • Attend all case file, staff meetings, training, and retreats.


QUALIFICATIONS

  • Bachelor’s degree or equivalent experience
  • Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)
  • Working knowledge of social services agencies in Santa Monica and LA county agencies serving the homeless
  • Excellent human relations/communication skills; ability to act in a professional manner regardless of circumstances.
  • Skilled in non-violent crisis intervention
  • Ability to self-motivate and organize time effectively
  • Computer literate; able to use computerized database system for information management.

Work Environment

  • Combination of either field and/or office environment depending on position
  • On occasion walk or drive to different agency sites
  • May/Will necessitate working in busy and loud environments depending on position
  • May/Will be exposed to elements like cold, heat, dust, noise and odor depending on position
  • May need to bend, stoop, twist and sit throughout the day
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
