TITLE: Community Case Manager
DEPARTMENT: Community Support Program (CSP)
REPORTS TO: CSP Case Management Facilitator

PRIMARY PURPOSE:
Case Managers are responsible for coordinating and assuring the continuity of services for each patient on their caseload. This includes working with each patient and caregiver(s) to coordinate care planning, complete a comprehensive assessment of needs, develop a Case Management Plan, monitor the effectiveness of it, plus ensure resource linkage, referral, and advocacy. May also coordinate care and maintain the medical record within the agency.

DESCRIPTION OF DUTIES & TASKS:
Under the general supervision of the CM Facilitator and within the Center’s policies and procedures, the Community Case Manager performs the following functions:

ESSENTIAL FUNCTIONS:

  • Meet with patients/caregiver(s) (face to face) at intake, periodically and annually to obtain information to conduct a comprehensive assessment of needs.
  • During periodic face to face meetings revise Case Management Plan to document any newly identified needs and /or services.
  • Promote and ensure a person-centered planning approach to care and patient choice of services.
  • Complete all documentation and billing in compliance with standards set by state and agency policy.
  • Attend meetings, trainings, and in-services as required and participate in regularly scheduled supervisions as required.
  • Review records, summaries and assessments from current providers.
  • Link patients to services (including phone calls, scheduling appointments, verbal and written referrals to internal/external providers and the completion of associated documents) as needed
  • Communicate and coordinate with patient/caregiver(s), and providers/collaterals, as needed.
  • Review and monitor linkage to services identified in the Case Management Plan
  • Review needs of patient with internal/ external providers and any other involved parties
  • Attend clinical meetings to discuss patients as needed.
  • Meet quarterly with internal providers to obtain information to complete Quarterly Service Review documentation.
  • Attendance is an essential function of this position.

REQUIRED SKILLS AND ABILITIES:

  • Is able to establish a rapport with patients. Treats patients with dignity and respect and takes a patient centered approach treatment, as noted through supervision, team participation, and feedback from patients and staff. Is able to build a relationship of trust, as observed by patients, supervisor, and team.
  • Provides input into program issues and shares/receives feedback in a healthy, constructive atmosphere.
  • Has general knowledge about medications and diagnosis as well as a working knowledge of issues, rehabilitation and supportive living concepts associated with adult patients and uses these appropriately with patients.
  • Has working knowledge of crisis intervention techniques and counseling principles and remains current in EBP’s provided to patients.
  • With the assistance of the supervisor, is able to organize and prioritize a workload so that rehabilitative and administrative duties are completed on time and in accordance with productivity and quality standards.
  • Follows through with responsibilities to the satisfaction of contact persons and/or supervisor.
  • Learns the philosophy and operating procedures of the agency and applies them effectively both within and outside the agency.
  • Represents the patients/program/agency constructively both within and outside the agency.
  • Maintains general knowledge of community resources and presents these to patients/supervisor/team as appropriate.
  • Maintains a working knowledge of the benefits systems and utilizes this information to the best advantage of the patients.
  • Ability to use computers and word processing equipment.
  • Ability to express or exchange ideas by means of the spoken word and to receive detailed information through oral communication. Primary functions include activities in which verbal communication with patients and with others is key to job effectiveness. Visual acuity must meet a minimum standard to work with preparing, reading and analyzing data and reports, work with computer terminals, etc.

QUALIFICATIONS:
Education: Bachelor’s in mental health counseling, social work, psychology, nursing, counseling or other related field as specified in He-M426-11 and He-M426-13. Meets qualifications to provide Functional Support Services.
Experience: One to two years of experience with emphasis in mental health.
Licenses/Certifications: Must have and maintain a valid NH driver’s license and Liability Insurance.

WORK SCHEDULE:
Generally works Monday through Friday with occasional requirements to cover emergencies after hours.

SALARY:
This is a full time non-exempt position with hourly compensation per Lakes Region Mental Health Center’s compensation policies.
