TITLE: Oncology Social Worker

DEPARTMENT: Oncology Service Line

REPORTS TO: Oncology Ancillary Support Services Manager

SUMMARY: To assist patients and families to identify social, emotional, and environmental needs and to connect them with available resources/services through the provision of a full spectrum of discharge planning and social work services in the outpatient setting.

JOB QUALIFICATIONS Education/License: Social Work degree. Clinical Social Work license required or must be obtained within one year of hire. Experience: Minimum of three years oncology experience preferred, relative to the patient population Effective English communication skills, both written and oral. Computer literate; Microsoft office competency required.

PHYSICAL AND MENTAL FACTORS Physical: Occasionally requires light physical effort as in periods of standing at files or work tables, repeated changes of position within the work routine or prolonged sitting. May require travel between system campuses, private offices. Etc. Mental: Work demands and irregularity of hours create considerable inconvenience for home and social life planning, such as frequent availability; or location imposes frequent interruptions and distractions that are bothersome and not inherent in the work, and the work requires frequent substantive involvement in stressful situations causing considerable strain routinely
