Clinical Supervisor – Munson Healthcare – Cadillac, MI

SUMMARY

The overall responsibility of a Home Care Clinical Supervisor is to provide leadership and support to field staff ensuring that Home Care patients receive excellent care that is supported by best practice in the most efficient and fiscally responsible manner. The ideal candidate is a proven self-starter with the ability to work autonomously with minimal supervision.

ENTRY REQUIREMENTS

Graduate of an approved school of nursing with a Bachelor of Science in Nursing or graduation from an approved school of Physical or Occupational Therapy required.

Master of Science in Nursing or Master of Science in Physical or Occupational Therapy preferred.

Two (2) years of Home Care or Hospice experience required.

Licensed as Registered Nurse and in good standing with the Michigan Department of Licensing and Regulation or licensed as a Physical or Occupational Therapist in the State of Michigan and in good standing.

OASIS certification preferred.

Knowledge of appropriate Conditions of Participation and reimbursement required; CHAP standards, denials, and queries preferred.

Possesses current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices in various weather conditions.

Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.

Meets MHH established requirement for proof of health status. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or transfer assistive devices to avoid manipulating more than 35 pounds.

ORGANIZATION

Reports to the appropriate unit manager. Must develop positive, collaborative working relationships with patients, families, staff, physicians, and members of the community.

SPECIFIC DUTIES

1. Supports the Mission, Vision and Values of Munson Healthcare.

2. Embraces and supports the Performance Improvement philosophy of Munson Healthcare.

3. Promotes personal and patient safety.

4. Uses effective customer service/interpersonal skills at all times.

5. Oversees and directs the patient care services and clinical activities of the field staff.

6. Conducts IDT meetings, individual case conferences, and one-on-one conferences with staff.

7. Addresses day-to-day customer service concerns.

8. Performs record reviews as established by the quality plan, focusing on patient care needs and regulatory requirements.

9. Participates in the interview process. Is responsible for hiring and disciplining employees and performs annual performance evaluations including annual competency assessments.

10. Maintains clinical knowledge base of current patient population.

11. Coordinates new client referrals, assists with scheduling and problems related to staffing while ensuring continuity of care in the home setting.

12. Identifies and addresses aberrant practice trends both in the field and in documentation.

13. Oversees and directs individual staff educational and orientation needs in collaboration with the Education Department.

14. Works with payors and case managers to negotiate visits then reporting clinical updates as required and referring patients to other departments as appropriate.

15. Participates in quality improvement process including the collection of data as outlined in the Quality Plan.

16. Complies with MHH and department policies, procedures and guidelines.

17. Participates in after-hours supervisory coverage as assigned.

18. Maintains computer proficiency in the basic functions of the programs used by MHH, including e-mail and use of the personal calendar.

19. Demonstrated ability to communicate effectively in both orally and in writing and in planning and conducting presentations.

20. Performs other duties and responsibilities as assigned.
