Community Psychiatric Support Treatment Provider/ Case Manager – Life & Purpose Behavioral Health – Marietta, OH

Looking for a full time CPSTP/case manager to provide case management services and care for Mental Health & Substance Use Disorder clients. The ideal candidate would possess an Associates Degree in Mental Health Technology or equivalent education and experience.

Requirements: two-year associate degree or commensurate experience; wellness coach, peer support specialist; certified tobacco treatment specialist, health educator or qualified individual (e.g. community health worker with associate degree).

Must have valid driver’s license, clean driving record and reliable transportation. Basic First Aid, CPR, Handling Aggressive Behavior, Safety Practices, Cultural Competency, and Client Rights training is also required but may be obtained after employment.

Duties include:
1. Coordinates services for clients participating in case management services, counseling, med/som, partial hospitalization and residential programs. Assist clients when problems develop, and refer identified concerns to local community support systems, the counselor/social worker and/or supervisors. Implements and participants in programs for clients to include social skills activities, community resource, accessing self-help skills, parent education, facilitating groups, and overall treatment of the client. Assist clients applying for funding sources.
2. Responsible for documentation and completion of case records, reporting on cases, assuring completion of intake and termination reports, preparing treatment plans, keeping progress notes and other record, as necessary.
3. Consults or represents agency with courts, social agencies, and schools as assigned by supervisor. Will complete clinical supervision in accordance with agency policy and procedures as well as regulatory bodies.
4. Will assist in assuring program compliance with board and administrative directives as well as state and federal regulations and administrative supervision policy and procedures.
5. Other duties as assigned by administrative and/or clinical supervisor.

Minimum Requirements: Associates Degree in social services related field; Bachelor’s degree in social service-related field and prior experience preferred.
