Encore Healthcare and Rehabilitation, a person-centered, long-term care community in Malvern, is currently seeking an LPN for the MDS Coordinator position. This is a full-time position, working Monday through Friday.

If you are currently an LPN looking to grow your career in long-term care, we have just the job for you! The MDS Coordinator position is responsible for the completion, review, revision, and update of Care Plans and MDSs per Medicare, Medicaid, and facility policies and procedures.
MDS Coordinator Requirements:
- Knowledge and skills in electronic charting
- One-year experience as an MDS coordinator in a Skilled Nursing Facility is preferred
