Monday – Thursday; Hours vary each day, and are between 8-30am-7:30pm.

A Scribe is an associate who enters patient information into the electronic medical record under the direction of a provider during the performance of a clinical service. This staff member follows the provider and is present during the provider’s performance of a clinical service. The individual documents (on behalf of the provider) in real time the patient experience and information shared during the visit. The individual may not be seeing the patient in any other clinical capacity and must not interject his/her own observations or impressions. Tasks performed by the scribe must adhere to approved standards of care in compliance with CMS rules, the Joint Commission standards and NCH guidelines. A scribe is considered an Unlicensed Assistive Personnel (UAP).

Responsibilities:

1. Records details of the patient encounter in the electronic medical record completely and in real time for ambulatory care providers. The scribed note must accurately reflect the services rendered on the specific date of service.

2. Maintains patient confidentiality according to organizational policies and HIPAA standards.

3. Applies knowledge of electronic health record documentation and medical terminology to enter notes for the patient encounter per provider direction.

4. Transcribes encounters within or exceeding departmental productivity and quality standards.

5. Records history of present illness, presenting problems, medical and social history, diagnostic criteria, treatment notes, treatment plan, and general patient condition per provider direction.

6. Verifies patient information for accuracy and completion.

7. Enters orders which will be authenticated by the provider prior to release.

8. Assists in disposition of patient and provides the after visit summary and treatment plan to the clinical associate or provider.

9. Notifies the provider of returned results and checks progress of testing.

10. Completes documentation prior to leaving the patient care area.

11. Uses interpersonal skills effectively to build and maintain cooperative working relationships.

12. Works and communicates in a positive manner with providers, clinical associates and leaders.

13. Contributes to the team effort, assists coworkers proactively to maintain efficient operations.

14. Participates in staff meetings and department problem solving.

15. Maintains knowledge of EMR upgrades and completes ongoing training requirements and competencies.

16. Maintains knowledge of scribing procedures and standards.

17. Follows and demonstrates knowledge of policies and procedures to maintain efficient and accurate documentation practices, seeks clarification if needed.18. Assists in orientation and training of associates as requested and competent to perform education.

Qualifications:

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

1. High School diploma or equivalent required.

2. Associates degree desirable.

3. Successful passage of an NCH administered typing test required prior to hire.

4. Previous experience in a medical or pediatric setting desirable.

5. Successful completion of Unlicensed Assistive Personnel orientation, and skills verification as provided by Nationwide Children’s Hospital.

6. Working knowledge of medical, mental health and/or pediatric terminology and accepted abbreviations. Computer familiarity and literacy with general data entry. Ability to learn and be proficient with specific computer programs, systems and Electronic Medical Record documentation.

7. Good communication and collaboration skills and ability to coordinate and cooperate with multiple members of the healthcare team.

8. Professional demeanor with patients and staff and recognition of privacy considerations for patients and families.

MINIMUM PHYSICAL REQUIREMENTS:

Note frequency as Constantly (67-100%), Frequently (34-66%) and Occasionally (0-33%) for each requirement.

Also note any lifting requirements with pound limit.
