ROLE SUMMARY:

In this role, the Behavioral Health Specialist will collaborate with an interdisciplinary Care Team composed of primary care providers, psychiatric and allied health professionals, and community health workers to manage the behavioral health needs of an assigned panel of patients.

DUTIES AND RESPONSIBILITIES:

  • For all assigned patients in your panel, conduct initial behavioral health consultations and screenings within 30 days of enrollment, plan and direct individual patient treatment programs, and serve as an advisor to other members in the care team on each individual patient’s behavioral health needs
  • Devise and implement treatment plans that include evidence-based mental health, substance use disorder, and chronic disease management interventions to meet the psychosocial needs of assigned patients/families
  • Provide psychotherapy interventions to assigned individuals, couples, families, and groups via phone, video or chat using our software platform
  • Conduct psychosocial screening and track screening results over time to ensure intervention efficacy and patient progress
  • Provide scheduled and unscheduled virtual consultations as needed for psychological
  • Conduct social determinants of health assessments and referrals to meet social needs
  • Collaborate and partner with clinical and non-clinical care team members to manage a patient’s health needs holistically; champion the integration of behavioral health and social-emotional wellness into the model of care for each patient
  • Attend weekly interdisciplinary care team conferences to select, plan, and evaluate care plans and treatment programs to ensure appropriate care and follow-up of assigned patients
  • Consult with medical, nursing, and psychiatric staff regarding diagnosis, strengths, and deficits as determined by psychological and social determinants of health assessment and evaluation
  • Instruct and counsel patients and their families regarding compliance with prescribed therapeutic regimens and adherence to prescribed medication regimens, within the scope of practice
  • Implement specialized therapeutic frameworks such as ____ as needed
  • Develop and conduct psycho-educational classes and groups; provides appropriate support to patient’s family
  • Conduct health assessments including but not limited to assessment administration, interpretation, and recommendations
  • Work with the patient’s primary care provider, specialists, hospitals, diagnostic services, pharmacy services, clinics, and community-based organizations to identify and link patients to services and supplies which can improve the health status of the patients
  • Participate in the identification, development and implementation of community-based partnerships and initiatives championing the integration of behavioral health
  • Provide medication reconciliation services and direct reminders to patients via phone calls, messages, and virtual visits to increase compliance with their treatment plan, medication adherence, and address any gaps in care
  • Perform post-discharge and transitions of care behavioral health interventions for members of your patient panel within prescribed times following discharge from a facility
  • Prepare intake summaries, treatment plans, and case summaries; maintains ongoing confidential records
  • Chart patients’ treatment and progress in accordance with company policies, regulations and guidelines and in keeping with accepted community standards
  • Meet clinical and operational service delivery targets on a monthly, quarterly and annual basis
  • Perform other related duties as assigned by management.

QUALIFICATIONS:

  • A valid, active LCSW license in the state of Illinois
  • 2-4 years’ experience working with patients with both chronic medical and chronic behavioral health conditions
  • Experience in an interdisciplinary team setting with a focus on integration of behavioral health and community health into primary care a plus
  • Demonstrated interest in combating systemic racism in health care and serving vulnerable communities with complex care needs; experience working with
  • Demonstrated interest in leveraging technology to drive equitable outcomes in health care; interest in healthcare data analysis a plus
  • Experience working collaboratively with social workers, community health workers, or care coordinators a plus
  • Experience in a specialized field (e.g. substance use, trauma-informed care) a plus
  • Knowledge of state regulations and standards with regard to ethics, treatment, patient rights, and confidentiality
  • Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.
  • Strong charting/documentation skills
  • Ability to effectively communicate with people at all levels and from various backgrounds
  • Multilingual skills a plus
  • Good judgement with the ability to make timely and sound decisions
  • Strong organizational, problem-solving, and analytical skills
  • Ability to manage priorities and workflow
  • Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm
  • Ability to travel locally to conduct home-based visits
  • Will require working evenings until 9pm CST
  • May include weekend coverage as needed
