Coastal Horizons Center, Inc. is seeking a qualified Care Management professional for our TASC Justice Services Intervention Program. We have this specific opportunity located in Nash County, NC.
Primary duties include administering psycho-social assessments/screenings, providing referral for services, and providing professional care management services for the justice-involved population.
Qualifications / Skills: Four (4) year college degree in human services or related field, combined with one (1) year of work experience within the judicial system or the substance use field or Two (2) year college degree in human services or related field, combined with four (4) years of work experience within the judicial system or the substance use field.
Salary: Salary Range for this position is $36,000-38,000, commensurate with experience, medical/dental insurance, life insurance, disability insurance, retirement savings plan/401K, paid time off programs.
