- Licensed in Michigan as Registered Nurse (RN) with no restrictions
- Verification of OIG/SAM/ Background Screen/ Criminal History
- Provide recent tuberculosis screening on file (within 1 year)
- Provide proof of current CPR training
- Home Care Service and Geriatrics experience is required
- IV Therapy Certification or Phlebotomy Certification required with multiple years of experience.
- Own a personal laptop or desktop with reliable internet connection to complete the vendor’s initial online training
- Valid driver license and car insurance
- Ability to travel within applicable state or locality with reliable transportation up to 35 miles from home address
RESPONSIBILITIES
* Complete Initial Credentialing Process
- Complete Vendor’s Training (4.0 hrs – paid training)
- Perform multiple in home visits per week with patient
- Record patient’s medical history accurately (medications and hospitalizations)
- Take and record measurements of blood pressure, temperature, heart rate etc.
- Monitor patients’ condition
- Collect samples for lab testing as assigned
- Perform additional task per assigned visit requirements
- Complete visit documentation and return via fax or email with 24 Hours
- Communicate with patient, practice manager and vendor representatives using electronic devices (telephone call, email and text message)
Salary: $40.00 to $75.00 /day
