Our affiliated company, has an excellent opportunity for a provider experienced in the following specialty: Case Manager – RN for Home Health
· This opportunity will be available in Glendale, CA
· The facility needs an experienced Case Manager with Home Health background to be part of their team.
· Rates is $40-50/hour (depends on experience)
· Your schedule or shifts for this assignment will be 9:00AM-5:00PM Monday-Friday
Job Summary:
- Check active cases/patients.
- Check clinical assessments.
- Contact via email or call if notes or oases is missing.
- Route sheets
- Knowledge on DSL (company has DSL as EMR software) but knowledge/experience on other systems/software is a plus.
Initial Requirements:
- Professional License
- Basic Life Support Certificate or Active CPR Card
- Driver’s License
- Auto Insurance (registered to the applicant’s name)
- Physical Exam or “Fit to Work” doctor’s note
- PPD or TB test result
- Flu Shot recordCase Manager and Home Health Experience
