Responsibilities include:
Provide assistance in maintaining or improving functioning when necessary to help a client achieve a rehabilitation goal as defined in the treatment plan
Assist clients in gaining access to needed medical, social/behavioral, educational, and other services and supports
Provide community-based case management activities to clients or to the LAR on behalf of the client.
If applicable assists the licensed staff in co-facilitating group psychosocial skills training to recipients, as assigned
Participate in staff training assignments, as requested
Assess, review, monitor a recipient’s status in relation to rehabilitation
Submit accurate, legible and complete client progress notes
Participate in the agency’s performance improvement initiative
Function well in a crisis
Qualifications:
Bachelor’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
Completed a standardized training curriculum
CANS and ANSA certified or willing to obtain certification prior to contract commencement
Demonstrated competency in the work to be performed.
Valid Driver’s License
Sharenote and CMBHS familiarity.
If hired should be available for work as soon as possible.
